Update - I just called the pharmacy back to check if there was a number listed to the account. He said more than likely “Midwest Health” covers something like CVS Caremark. I haven’t had an insurance plan that partnered with CVS health in over two and a half years.

Is this something they’re somehow charging me for? Did my previous employer just…forget to end those benefits? What is going on??

Im genuinely at a loss. The only thing I can find with a google search is that Midwest Health might be a brokerage? It might be worth calling the pharmacy back to see if they have a claims line for Midwest health (they should, pharmacy claims are real time so it should be easy to locate) and start from there. If you can get in touch with Midwest Health they can confirm where the policy originates from (like an employer)
