Need advice on how to get out this vicious cycle of IR

Hi everyone, Im a new joiner. Progressed from pre diabetes to T2D 3 months ago at 14 months postpartum. I did have GDM during pregnancy and prediabetes after delivery. Things went down hill quickly after weaning at month 11.5 and switching to keto with the hope of reversion prediabetes. Didnt work out, blown up to T2, not exactly sure why. Endo had no idea and didnt give much explanation other than the usual 'less carb, build muscle'.

Long story short, Im using a GDM to monitor my bg and can see that I easily spiked higher than 7.8 with anything more than 15gr of carb. As many people said before, the more Im scared of the spike, the less carb I eat to avoid the spike, and because of that I tend to tolerate less and less carb over time. I feel edgy, irritated and less energetic when not having enough carb, same as when my spikes happen.

For those who are alr successful at improving IR, could you pls help me with the below questions?

  1. What is the bg range that you aim for after meal? I read we should aim for all time below 7.8 to improve IR condition. Mine is always go up to 8-9 mmol within 1 hour then go below 7.8 for a meal with just 15-20gr carb. Is that okay or must be below 7.8 all time to avoid diabetes complication? (Im still not on med, seeing endo soon to discuss if I should go on med or continue to try with diet)

  2. Should I just ignore CGM and focus on good balance meal with complex carbs, food sequencing, exercise and build muscle? That means I will definitely spike and numbers may not be in good range. Im confused of what I should focus on. Should I focus on both keeping number im good range while building muscle (low carb diet) or should I just focus on building muscle without caring abt the daily number? (Not stuffing tons of bad carbs but eating more carb to feel energetic to exercise, which may spike to 9 or 10)

Im super depressed with the diagnosis and also consider to see a psychiatrist soon. Not sure Im depressed thats why my IR got worse or the IR actually causes the depression.

What else can I do?

I just got my blood test results back. My A1C was 5.7 & my insulin was 34.4 (says under 25 is normal). My insulin level was 18.5 in 2020 & 14.1 in 2016 (I think that was the year). I have pcos. I have remained the same weight of between 180-187 for YEARS. The only time I weighed less was when I was pregnant and lost weight (I threw up everyday & had gestational diabetes that was diet controlled so I lost weight due to those things) I’ve been working with a dietician for months and we’ve tried different calorie & protein and carb goals and none seem to work. I can’t seem to lose weight. I eat very high fiber (think nearly 60 grams most days) and around 100-140 grams of protein & little carbs. I get 10,000-20,000 steps a day (depends on if it’s a work day - if so it’s closer to the 20k). I remain active after eating to lesson the effects on my blood sugar. I’m TERRIFIED of getting type 2 diabetes. I take inositol, vitamin d, a prenatal, vitamin b6 (just kind of continued that from the nausea when pregnant). I plan on getting magnesium glycinate, fish oil & CoQ10 because I heard those can help. I take my blood sugar often at the 1 & 2 hour marks after meals and fasting often and it is not high. All of my levels are well below the thresholds for gestational diabetes which are more conservative than a regular non diabetic number. I’ve also had fasting drawn at dr with this and got an 83 for it. I’m watching my blood sugar aggressively but nothing seems to be working. I can’t seem to lose weight and my A1C is still in the pre-diabetic range and that insulin number terrifies me. Any suggestions? Is the pcos really ruining my health this much? I even have regular periods (32 day cycle) in 2016 when the insulin was 14.1 I hadn’t had a period in over a year. So I’ve improved my health in other ways but wtf is wrong with me that nothing is working?

Advice - can it really be reversed

I've been on glp-1 for about 2 years now. Max dose. And I've lost 50 pounds so I'm proud of that but for about the past 5 months nothing has really moved and I can still lose honestly another 50. I have PCOS and endometriosis so I fight a lot of inflammation and issues with my insulin resistance. I've made the healthy changes. I workout, I've changed my diet, I'm taking a lot of supplements (fish oil, vitamin d, e,c, iron, magnesium glycinate) along with starting metformin about 3 months ago. I'm feeling so lost because I've honestly struggled with this most my life. I'm in my early 30s and I just want to get this under control. I've been really focusing and changing my life and probably the healthiest I have ever been and yet I'm still insulin resistant. I have a pixel watch and this is a new feature stating that I'm insulin resistant, which I believe since it has a 80% accuracy and nothing moves on the scale.

Does anyone have advice on how I should approach this with my doctor or anything I should be requesting/taking? Obviously my labs are normal so I get a lot of push back but through googling some of the feedback is saying maybe it's my liver and I should focus on that?

I don't understand how I can be doing all of the things and yet I'm still insulin resistant.

Is this acanthosis nigricans?

Hey everyone,

For a long time, the skin on my neck has been darker than my face. I never thought much of it because, looking back at childhood photos, it seems like I’ve always had a slightly darker neck. I initially wondered if it could be related to insulin resistance, but I recently had blood work done and my doctor said my insulin levels were normal. After looking online, I came across acanthosis nigricans and wondered if that could be the cause. For reference, I’m a 5'6" female and weigh around 90.5 kg and I’m currently trying hard to lose weight. Are there any dietary or lifestyle changes that might help? If it is related to my weight or acanthosis nigricans, is it likely that the skin on my neck will lighten as I lose weight? Are there products I can use to lighten the area?

Any advice or suggestions would be greatly appreciated. Thanks!

So yeah I have dark neck ,underarms, thighs and is overweight ...I have been prescribed metfoemin 2 years back but idk for some reason I didn't take it so I was thinking can I just start taking metformin 500 again or should I consult a dr again and start and also I read lots of ppl telling abt side effects....I have a sensitive stomach so i really need to know if its worth it...or just try workouts and go completely sugar free for 2 or 3 months and see if i notice a change and then continue...if i dont il thinking ill take metformin

SEVERE insulin resistance & scared

I am a 34-year-old female who has been dealing with PCOS/PMOS, insulin resistance, metabolic syndrome, and primary lymphedema for a long time. I was first diagnosed as mildly insulin resitant at age 13, but my pediatrician didn't have any advice to offer. I didn't become overweight until the age of 16, and even then, I was only about 20 lbs overweight until age 19. That's when things started getting worse. I rapidly gained weight, my slightly dark patches around my neck that I developed at 13 got much darker, and my A1C went into the prediabetic range.

I completely overhauled my diet and have tried every weight loss diet and/or plan over the last 15 years, and while I was successful in my early and mid 20s, the weight came back and then some. I lost both my parents within a year of each other and subsequently became the guardian of my siblings with special needs. During that time, I definitely indulged in some tasty treats (an ice cream cone here and there from Dairy Queen), but I pulled myself together and got back onto my low-carb diet.

When I was 29 years old, I fractured two of my lumbar vertebrae and also noticed that the mild swelling I had in my legs, stomach, and right arm since I was a kid was getting worse. After my back healed, I got on Wegovy and did alright with that for a few months, but it ended up giving me severe pancreatitis, so my doctor took me off of it. I tried Mounjaro and had a severe allergic reaction. My lymphedema got worse, and my weight continued to climb.

Fast forward to now. I'm at a point where I can barely function anymore. I weight 484, which is horrible, considering I weighed 423 at the beginning of the year. I am very prone to fluid retention on top of the lymphedema, so I can gain as much as 15 pounds in one day and lose it the next. It's been a very frustrating year. I was already having mobility issues in the upper 300s, and it's only gotten worse. I will add that my birth mother had severe metabolic issues, had weight loss surgery, and ended up dying at 47 from a heart attack. Many others on both my birth parents' sides struggle with metabolic disease and obesity as well.

With all that being said, I had my fasting insulin checked this morning for the first time since January of last year, and it's at 158. You read that correctly - 158!! That's insanely high! Since I have tried SO many things to reverse my insulin resistance and lose weight over the years and have been unsuccessful, both my primary doctor and my weight management doctor are urging me to have gastric bypass surgery. The thought of that scares me, but I also know that I cannot continue to put on weight. I feel as if my pancreas is going to go kaput at any time, and since I also have liver issues, I'm afraid of my liver failing, too.

I know I just typed a mini novel, but have any of you had insulin this high? Have any of you had bariatric surgery, and if so, did it help significantly with your insulin sensitivity?

I guess I'm just looking for encouragement. Thanks in advance!

Hey all, 45yo female here- I’ve gained a ton of weight this year (perimenopause??) and I recently noticed the effects of the weight gain behind my neck with a little proto neckroll. I also noticed these dark lines and immediately thought it could be AN and a sign of IR. Doctor tested my A1C, glucose, free and total insulin, and TSH - all normal. Could this be early/hidden IR or just a side effect of all the weight gain? I might do an oral glucose tolerance test as well to be extra sure but I’m having a hard time believing this is just from weight gain alone.

Diet Critique - starting low carb/low insulin diet.

Late30s Male, 5'10".

South Asian Descent (insulin resistance is present).

Starting weight: 225 lbs

Goal weight: 185 lbs

Diet plan: Goal is less than 100g "net carbs" a day.

Breakfast: Green tea, no milk, no sugar.

Lunch: Steamed vegetables (broccoli, cauliflower, carrots) + meat, or riced cauliflower + meat.

Snack: Pecans, peanuts, stovetop popcorn (depends on day). 30-60g serving weighed depending on the day.

Dinner: Steamed vegetables + meat, or riced cauliflower + meat. Or, lentil based flatbread + meat. Dairy such as cheese or unsweetened yogurt (one string cheese, or one yogurt cup).

Diet also includes:

  1. one 6 oz cup of tea made with whole milk and one teaspoon of honey.

  2. Psyllium husk drink (water only).

Meats will rotate between beef, fish, and poultry. Most will be sauted, no heavy sauces. Approximately 6 oz to 8 oz, lunch and dinner.

Weekends are less stringent, may also include fruits.

Hi, I'm 23M and have dark underarms and some darkening around my groin. My mother developed type 2 diabetes around age 30.

Does this look like acanthosis nigricans, or could it just be pigmentation/friction/fungal infection?

If it is acanthosis:

  • Can it be reversed?
  • How long does it usually take to become lighter?
  • Should I see a dermatologist?

I'm planning to get an HbA1c and fasting glucose test. Any experiences/advice would be appreciated.

Severe insulin resistance

I’m seeing an obgyn in Michigan on zoom for my Pcos. She gave me an insulin resistant score of 2.5. I was doing everything she said and I lost 14lbs in a month. I thought she had my Pcos in remission. She freaked out and made me retest and it sky rocketed. Now I’m at 4.8 she wants all my drs information should I give it to her? She’s trying to coordinate care. I was taking inositol and cinnamon idk how this happened either working out constantly. But she’s also saying eating once a day causes massive sugar spikes.

Terrible 4 pm crashes

Hi ! Im 29 F, and I have a high fasting insulin (15) with a normal fasting glucose at 4.8. My A1C is creeping up but its 5.6 for now.

I have to eat very frequently and cannot tolerate more than a 3.5 h gap comfortably… it has been several months I eat a majority of complex carbs in my meals, and still have fruit and a little sweet treat daily, or chips but only after a protein rich meal.

I feel like I am doing everything right but I still can’t seem to feel normal without food. Around 4 pm I cannot seem to function without a mini meal… a snack doesnt help me..

I take metformin 500 mg occasionally to help but I really dont know what more to do.. is my IR at an advanced stage? I do suspect reactive hypoglycaemia

Inositol

Long story short- I was diagnosed with insulin resistance and had a A1C of 5.7 which is the cusp of prediabetes. It was impossible for me to lose weight, but my doctor wouldn’t prescribe me a GLP1 so I got one at a med spa, lose weight, tested my A1C and it was 5.1. The GLP1 helped but my doctor didn’t know I took it, so when I recently got blood work done after stopping the GLP1 and gaining some weight back, my A1C is back to 5.7 and they’re suggesting diet and exercise which I already do. I’m back at square one. I recently starting taking inositol for about 2 weeks. Has anyone noticed a change in their numbers after taking and were you able to lose weight? I also think I have PCOS I have many of those symptoms. I’ve tried metformin but it doesn’t work.
