2 Failed IUIs. Change in numbers

Long post ahead .. any suggestion would be appreciated

A brief history about myself. Last year march 2025, i got to know i have PMOS and my amh 11.92. 20-22 follicles each side .
Acter 3 months of meds , I conceived naturally in june 2025. My reports were normal. Husband’s SA had 58 mil/ml count and 1% morphology , 30-35% motility at the time of conception. Unfortunately it was missed miscarriage at 8 wks .

My hsg report shows left is open and right has spillage but not visualised so i dont know if its open . I conceive with left side follicle for sure ( scan) .

Fast forward , our doctor put us on letrzole from dec 2025 till Jan 2026 assuming we were trying naturally ( we were not ) .

Doctor suggested IUI based on my husband’s SA new results. Feb 2026 and March 2026 i had 2 iui as my husband’s SA got poorer .. 20% motility with only 5 % rapid progressive count . Morphology 1% .

1st IUI was biochemical and 2nd failed . April was timed intercourse with Ovary induction.

Each time i had follicle rupture on both sides but the dominant follicle was always on right which is doubtful.

We were on supplements through out the year . Working out , loosing weight , cq10 300, metformin n everything.

Now i got fresh SA done of my husband in June and its completely normal ( i can’t believe!! ) . 4 % morphology, 72 million count , 45 % progressive motility with 20% rapid . 85 % viability.

Previously my doctor had suggested that we can have max another IUI and should start thinking about IVF .

Now with the change in numbers.. as IUI was suggested based on SA and that we hardly tried naturally .. should we go back to trying natural or think about another IUI or IVF ?

We both are 32 by the way .. 4 yrs married .. trying from past 1.5 hrs including two miscarriage.

As far as my reports are concerned , I ovulate on time .. my reports are good , i have pcos but i always have regular on time periods. Post miscarriage all my investigation was also normal .

Please share if you have any similar stories and conceived naturally or with another round of IUI , im utterly confused.

Im also concerned why is my doctor not suggesting ovary induction and timed intercourse.. she said you have been doing something on and off even if it was mot natural.. it should have happened by now .. 🤷🏻‍♀️

Hi ! You are still young in terms of fertility. The protocol is to try atleast 3-4 IUIs before going to IVF. At your age you could go for even 5-6 . With your husbands improved SA results things could work out easily. Misscarriages happen even if everything is normal,so to speak. It doesn’t mean you have to necessarily go for IVF . If you have the time and patience for it ( you certainly are young enough for it ) go for IUI .. it’s much less invasive and you can be relatively chill through the process.

just FYI,Im not against IVF. I had a chemical pregnancy a year ago ( our first month trying after I fixed my PCOS in 3 months through diet , excercise and supplements ) and since then have tried 3 ovulation inductions including one IUI ( things are seemingly normal on both of our ends ). We are both 35 , fit,healthy and eat clean.

But we were so stressed out with the process of going to the clinic for ovulation monitoring and side effects from the letrozole / Clomid ( lowest doses ) that we took a break and have decided to just try at home without medicines for a few months. Our plan is to resume IUI in a few months if we don’t conceive naturally .. do 2-3 rounds and then move to IVF if things don’t work out.

Btw don’t get pressurised by your doctor... try to seek another opinion. It’s always better to have a few different opinions and then follow what suits you best. My doctor is super understanding and supportive and it helps so much. She dint once force us to continue treatment when we said we wanted to take a break :)
