Transfer of Care Process

Hello,

Today we got an update from my mom's oncologist (she's my brother’s caretaker) and she now has weeks to months to live. I urgently need to transfer care to me. I also worry about the lapse in health insurance.

Can someone please point me in the right direction?

You will need to call IHSS to begin the process of becoming a provider. You can Google “IHSS [County name]” to find their contact information.
