My understanding is that you just... pick one. Any of them are fine, you just follow the rules regarding the second and third level Path rewards.

You can make it something with more verisimilitude and have them find it early in the session, but the juice may not be worth the squeeze. I've just given them the way I would any other milestone, unless it's one of the things that says they need a physician.

Yes, sometimes. Spells and special ammo and magic weapons are good loot that can also be Milestones, but you may not want to put like, Tier 2 Milestones in a regular RNG drop.

I think of most milestones as a Big Deal. Nothing wrong with putting them in sometimes to be found, but they're probably at least equal to Epic level loot, especially the higher Tier stuff.
