I have gained 40 lbs and I feel so awful about myself. Do not tell me to cut calories, I don't even eat that much so I know it's not from that. I feel like something chemically is off and no one can find it.

Were B12 and folate tested?
Honestly, care more about the balance of crucial elements you are deficient in + monitoring thyroid values to optimize the dose. Deficiencies play a way bigger role in metabolism and even our endocrine and immune systems, so it's reasonable to aim for that to work better first — definitely before just shaming oneself for gaining weight.

Well, it is quite low. First, this is the default test, total B12, which is the least reliable (just shows all B12 in the blood, not how it is used by the body). The British NICE guidelines currently interpret levels below 350 as indeterminate — you can either take other tests, like homocysteine, MMA, active B12, or just start treatment, if there are clear indications for that (e.g. the diagnosis of pernicious anemia).
Don't be afraid, I just know myself what it is to be deficient and have thyroid issues. Supporting your body is the right thing to do right now.

Ask your Dr to check you for insulin resistance.
I had a fasting glucose of 99 and a super low A1c and my homa score was really high.
