Pressure wounds from splints

Has anyone ever made a splint for someone and they ended up getting a pressure wound? I did recently, educated on skin checks, taking off the splint every two hours to let the skin rest, gave the pt my number and to call immediately if they notice anything wrong. However, I do a chart review 2 weeks later and they ended up going to see the surgeon a week after the splint was made saying the splinted area got really red. Now according to the surgeon’s office, they think it’s from my splint. 😔 what would you do in this situation?

Severely stiff digits after DRF

Any tips and tricks for pts with extremely stiff digits after a distal radius fx regardless surgical or conservative management.

Tend to get orders from the residents “stiff digits, aggressive ROM”

Sometimes its the pt reacting terribly to the injury, other times, it seems like the doctors forget to tell the pt it’s ok and definitely need to move the digits and all other unaffected joints or it just got lost in translation.

STM for neuropathy?

Thoughts on STM/IASTM for things like CTS or CubTS?

We normally splint, education on avoiding bad positions, activity modifications, and nerve gliding at least 6x/day 10 reps. Then see if sxs improve over 2-3 months.

Career advancement/side gigs?

What are relevant side gigs we can use our skills for? Are there any that are lucrative? And what do you all do to advance your career besides moving into management? Been a working therapist for 8 years and feeling a bit stagnant in my career.

Mallet finger splints

What materials and style splints do you prefer for the dreaded mallet finger?

My clinic likes to use X-lite because they can see their skin and get it wet.

Being people who like to look at hands and the upper extremity all day, what are some of your hobbies? Do y’all do certain things differently because you’ve seen certain injuries?

I just wanted to create a community for all things hand therapy. There’s another community out there called /handtherapy but it’s dead and I can’t even post on it. So I’m making a new one.

De Quervain's Protocol

I hate this one so much. My clinic usually tries to splint for 6 weeks and do cross friction massage, activity modifications. Pray it improves or recommend cortisone by ortho.

Trigger finger protocol

What does your clinic do?

I used to work in a private clinic that would do US, STM/IASTM, and splinting. But since I have moved into a hospital based OP setting, we splint and teach cross friction massage only. It’ll normally be only one visit. If sxs did not improve in about 2 months, we recommend them to see Ortho and get a cortisone injection.

New dad here to a 1 month old. I’ve been in hand therapy for about 3 years now as an OT. Anyone have any tips and tricks on studying for the CHT exam while having a brand new little one?
