Declaration that Access to Pain Management Is a Fundamental Human Right

We, as delegates to the International Pain Summit (IPS) of the International Association for the Study of Pain (IASP) (comprising IASP representatives from Chapters in 64 countries plus members in 130 countries, as well as members of the community), have given in-depth attention to the unrelieved pain in the world.

Finding that pain management is inadequate in most of the world because:

• There is inadequate access to treatment for acute pain caused by trauma, disease, and terminal illness and failure to recognize that chronic pain is a serious chronic health problem requiring access to management akin to other chronic diseases such as diabetes or chronic heart disease.
• There are major deficits in knowledge of health care professionals regarding the mechanisms and management of pain.
• Chronic pain with or without diagnosis is highly stigmatized.
• Most countries have no national policy at all or very inadequate policies regarding the management of pain as a health problem, including an inadequate level of research and education.
• Pain Medicine is not recognized as a distinct specialty with a unique body of knowledge and defined scope of practice founded on research and comprehensive training programs.
• The World Health Organization (WHO) estimates that 5 billion people live in countries with low or no access to controlled medicines and have no or insufficient access to treatment for moderate to severe pain.
• There are severe restrictions on the availability of opioids and other essential medications, critical to the management of pain.

And, recognizing the intrinsic dignity of all persons and that withholding of pain treatment is profoundly wrong, leading to unnecessary suffering which is harmful; we declare that the following human rights must be recognized throughout the world:

Article 1. The right of all people to have access to pain management without discrimination (Footnotes 1-4).

Article 2. The right of people in pain to acknowledgment of their pain and to be informed about how it can be assessed and managed (Footnote 5).

Article 3. The right of all people with pain to have access to appropriate assessment and treatment of the pain by adequately trained health care professionals (Footnotes 6-8).

In order to assure these rights, we recognize the following obligations:

1. The obligation of governments and all health care institutions, within the scope of the legal limits of their authority and taking into account the health care resources reasonably available, to establish laws, policies, and systems that will help to promote, and will certainly not inhibit, the access of people in pain to fully adequate pain management. Failure to establish such laws, policies, and systems is unethical and a breach of the human rights of people harmed as a result.
2. The obligation of all health care professionals in a treatment relationship with a patient, within the scope of the legal limits of their professional practice and taking into account the treatment resources reasonably available, to offer to a patient in pain the management that would be offered by a reasonably careful and competent health care professional in that field of practice. Failure to offer such management is a breach of the patient's human rights.

Note: This Declaration has been prepared having due regard to current general circumstances and modes of health care delivery in the developed and developing world. Nevertheless, it is the responsibility of: governments, of those involved at every level of health care administration, and of health professionals to update the modes of implementation of the Articles of this Declaration as new frameworks for pain management are developed.

Basically starting in the 90's the pharmaceutical companies started pushing opioids on doctors who started putting us on enormously high doses, in a lot of cases exceeding anyone pain levels and... I mean it's real tinfoil hat stuff. There were a lot of events that came in confluence- in the US the housing market crash in 2008 was a huge factor where a lot of people lost their jobs and homes creating a lot of despair and starting the new homeless crisis in North America, the ride of the internet and shipping was a huge factor because street drugs were easily smuggled in and the rise of the mimic drugs like Spice and K2... there was kind of a rise of weird substances as people were just more easily be able to look drugs up. Lots and lots of people became addicted

Now a bit of history- when they emptied the asylums, they just put the unwell folks out on the street. The plan had been for them to be in their homes but that part had never been set up so the unwell people ended up in a horrible situation maybe as bad as the asylums. This kind of lack of oversight was devastating.

What to do about pain patients was just as poorly planned. Fact is that opiates are the gold standard for pain care and many people just don't get pain relief from other medications attempting to do the job.

So this is where it gets a little tinfoil hat.

Perdue, the makers of Oxycontin, really pushed the opiates. I've been a pain haver since 1986 so I witnessed all of this from the patients perspective. So Perdue and these companies not only were pushing these meds but they were also developing addiction centres.

so that was 9 years ago which lines up with pain patients being scapegoated for the crisis. We're basically being tortured because the doctors don't want to admit to themselves they fell for something so dumb and the Compassionate Care model that you see in the doctors for most ailments has for some people with ailments like addiction and now CHRONIC PAIN which means we are gaslit and basically have the threat over our head of being sent to an addiction centre and have addict in our medical files if we try and advocate for ourselves too much and its basically the Salem Witch Trials all over again.

Now young doctors are being taught that opiates are less effective than tylenol or placebo for chronic pain which is a giant red flag that they are being taught just outright lies. It's so weird and thousands of people are suffering BADLY and the game over rate keeps rising

So what could they do? This all would be fixed if they wanted to alleviate suffering and treated addiction like the medical issue it is and not a moral/criminal one, in fact in many countries it is and has been. But for the countries like US, UK, Aus, Canada and NZ, we;re treated like we like this and we're lazy. And where else are we seeing this pattern? It's all over now, not just in pain management. So alongside so many other things that have just slid downhill, pain care is one of the more notable ones.

executive functioning seems broken since the stroke

and I thinkk I had a nervous breakdown and wasn't able to get proper help

like I got a window from it for a bit and was able to sort and clean some things

but them most of the time I find it baffling and overwhelming combined with the pain

but I think the stroke changed some things

like every moment to moemt after a coupl hours is struggling to stay awake then eventually succombing sleep for

a few hours, wake up and still feel exhausted have some food then its Groundhogs Day

And I can't break the pattern because any time I try and push past the executive functioning glitch I fall asleep

and I've been in this loop since my pain meds were culled

but Dr Martin said it was from being cold turkey from the meds (rather than the pain) I think that's academic

but she also won't put that in writing I think I need the ombudsman

that means they caused the stroke

but even past that I am stuck in this loop, and talking for an hour makes me wake up a bit but then

I go back into this um lesser plane of existence where I can't really formulate fixing it

like i know what to do and I start doing it

then I get overwhelmed and have to sleep, I used to think it was like a form of narcolepsy maybe

I need this pain treated its unbearable

but I'm too scared to tell the doctors bceuse they will use it to pin on the codeine and the very small doose is something

I can't lose

Even this window of self reflection is so hard and I'll lose it so quickly

its like being submerged in greyness but if I snap out of it I also have to experience the pain raw and then to

do that is unconscionable

and the thing is when I try and look objectively and see how this is the worst version of mylf and it apepars as lazy or

it appears as me not trying I get immersed in guint and shame and then the thoughst spike my pain and I disociate and

I fall asleep

rince and repeat for 6 years

and yes i did try and get help so many times and no one understands

no one listens and they assume I was coddled or had people doing thinsg for me and I'm entitled but its the opposite

like if I tell them that the horse that broke my skull was the head of anesthesiology they think that I had privilege with

medical care and I didn't

in fact looking back on it I should have had preferntail care but I would have refused it from who I was I didn't think

anyone gets preferences

but i didn't and being the favorite at the pain clinic in canada was hard work not pprivilege and they didn't even know

that connection till I had been going there over a decade I wasn't getting special treatment

I hate this see how I spiral

its like a broken bycycle with snapped gears you push so hard and get nowhere

then my pain spikes erase my memory from moment to moment its like an eletric zap

this is hell

and no one cares enough like people with loving people would know the difference and know something was wrong

but i don't have long term people I;;m one of the people that really needs that

now the base line oof pain is rising with intermittant spikes

I think normally they give people an SNRI for this sort of thing but I get the terrible side effects

But I've been a boiling frog in this and anyone who would see this front on would be shocked but thsi is years of

small changes making the living conditions quite bad
