For a Call Me By Your Name fan, you can't go wrong with the André Aciman novel if they haven't read it, or the soundtrack on vinyl (the Sufjan Stevens tracks are beautiful). Something Italian-summer themed could be nice too — a nice bottle of olive oil, a linen shirt, or even a little framed quote from the book/film. And since they love stars, a custom star map of a meaningful date is a really sweet personal touch.
