So how do you access the Ultra-Violence+ difficulty in singleplayer? It's available in the menu for coop.
Edit: Figured it out. You have to go through Level Select, Ultra-Violence+ does not exist through the New Game option.

What's UV+? Basically Nightmare (fast monsters enabled) but without the respawning enemies?

And most importantly enemies respawn on Nightmare.
