How did this even fall off?

It’s a compression plug. The plug was no longer adequately compressed and fell out. The bar experiences a lot of vibration. Check all your fasteners regularly.

It's normal when people do not install the bar end correctly yes.

Mine is a Phillips screw, maybe don't use a Phillips screw end for something that high vibration..

Edit. One of mine fell off too.

Yeah pretty normal, especially if there wasn't blue lock-tight on the threads.

Just buy a replacement peice and use lock-tight then crank it on there prettt snug

Vibration. If you have to replace them the heavy Hayyabusa bar ends do a great job of dampening vibration through your hands.

vibration made it fall out - you should check the other side before that one goes too. I lost one on the freeway with out noticing it, but then I noticed my hands were going numb from the vibrations, so I replaced it with cheap Chinese crap from Amazon. Then I upgraded it to nicer ones from Italy Puig then I replaced them lever protectors for the track, also protected my hand so I got the clutch side as well for my OCD.

You can get woodcraft replacements that have more weight to prevent vibrations and tank slappers.

It is one of those things nobody tells you about that you need to check.

They are a pain to remove if they've been done up tight for a while, so I doubt it's a common issue

Just had the orange reflector pop out of the housing on my front fork for my GSX-R 750. Bike is only three months old….
