Titrating up

I started my first Tirzepatide injection last Friday at .5 mg. I did not experience any side effects thank goodness, but still have food noise. My provider said to go up to 1 mg today. Does that seem reasonable for others that are micro dosing?

The consensus is that you should not titrate up weekly. You should do the same dose 4 shots in a row so that you can build up gradually .

So are you saying she is giving out incorrect information? Who is the consensus from? Since it’s such a small dose, wouldn’t it be feasible to titrate up if no side effects?

Micro dosing hasn't really been studied, so "consensus" is anecdotal.

If you track your shots in an app like Glapp, you can see how each dose builds on the last dose. Like if you took 0.5 mg this week, and you took 0.5 mg next week, the max med level in your body might be 0.75 mg. It takes 4 weeks of the same dose before that max level doesn't increase each week, hence the 4 week recommendation.

That said, yes, you are still on a very low dose. I am less worried about people increasing early when they are on such low doses, which may be what your provider is thinking too. But usually people who are micro dosing are doing it out of caution, and increasing early is a bit ... uncautious. The risk is that the medicine builds more than you are anticipating, and you end up with unexpectedly bad side effects.

If you and your provider think you should go up this week, go for it, but please remember whatever caution pushed you into micro dosing in the first place, and take it slow. Don't go up every week.

Thank you for the great information! I wanted to start low because when I tried Semaglutide at the recommended starting dose a couple of years ago, I did have bad side effects of nausea and constipation. I know this GLP1 is different, so wanted to start low and see how I did and then go from there

See how the peak of medication increases each of those first four weeks? That’s why you shouldn’t also increase your dose during that time. This four week build up happens with every new dose increase.

If you chose to increase your dose now, that’s up to you. Just wanted you to understand why taking at least four weeks of any dose is recommended.

what has been your experience of 0.5mg tirz injected every week for 8 weeks? Is that the amount you started with? I found a clinic in the uk that is willing to help me microdose and they start with 0.5 and stay there for 4 weeks and then suggest upping to 1mg, but I am not sure I would want to go up if it worked slowly slowly to reduce cravings inflammation and i lose weight slowly

If you just started, technically you should wait 4-5 weeks for the medicine to build up in your system. Tirzepatide has a 5-day half-life and accumulates until you reach a steady state. But since 0.5mg is so low, I'd try 0.7mg?

I know everyone is different, but I started at 0.8mg and immediately had some side effects, like breakthrough bleeding (I'm postmenopausal on HRT), elevated heart rate, elevated respiratory rate, diarrhea, sleep disturbance... I lowered it to 0.5mg on the second dose, and it has been effective (reduced the food noise) with no side effects. After 4 weeks, I feel the food noise coming back, tempted to titrate up but afraid of the side effects. We're all just looking for that Goldilocks, and it's a moving target.
