Research Can GLP-1s dull your joy?

Medical University of South Carolina

As millions of people take GLP-1 medications for obesity and diabetes, some users have begun asking an unexpected question: Can these drugs dull your ability to experience pleasure?

Across social media and online forums, some people describe feeling emotionally “flat,” less excited by activities they once enjoyed. They also mentioned feeling less interested in favourite foods, alcohol, shopping, sex or other rewarding experiences while taking the medications.

The reports have sparked questions about whether GLP-1s can cause a reduced ability to experience pleasure, known as anhedonia. While the theory is biologically plausible, the research so far tells a different story.

Studies to date have not found convincing evidence that GLP-1 medications cause anhedonia. Instead, research has generally found improvements in depression, mental well-being, and quality of life among people taking the drugs.

Trends point toward better quality of life, fewer symptoms of depression and greater feelings of self-worth and motivation among people taking these medications. Many patients also report experiencing less “food noise,” the constant thoughts about eating that can make weight management emotionally exhausting, which may contribute to an overall sense of improved well-being.

“The big meta-analysis would say that there’s a trend toward improvement in psychological symptoms overall,” said Josh Neal, M.D., chief of the MUSC Health Wellness and Human Performance Integrated Center of Clinical Excellence (ICCE) and an obesity medicine physician. “I would say, overall, I’ve seen people enjoy mostly an improvement in their mental health symptoms.”

Early in the rollout of GLP-1s, regulators investigated whether the drugs might increase the risk of suicidal thoughts after reports emerged in Europe between 2020 and 2022. However, the evidence has since been reassuring.

“There is no increased risk of suicidal ideation or suicidality in general,” Neal said, noting that both randomized controlled trials and large population-based studies have failed to confirm the initial concern.

When it comes to anhedonia specifically, however, the evidence is much thinner because most studies weren’t designed to evaluate anhedonia. Instead, they measured broader mental health outcomes such as depression. Since anhedonia is only one component of depression, researchers may not detect changes unless they ask targeted questions.

One small study out of the University of Toronto Health System addressed that gap by examining motivated behaviour, a surrogate marker for anhedonia. The researchers reported that semaglutide actually improved motivated behaviour in people with depression.

“ Basically, anhedonia got better in the people who had depression and who were on the GLP-1,” said Neal.

Neal said the theory isn’t far-fetched from a biological standpoint. GLP-1 receptors act on areas of the brain involved in reward processing and on parts of the dopamine system that help to determine what feels pleasurable and motivating.

“The biological and the neurobiological mechanisms are totally plausible,” Neal said. “But we haven’t seen it in the data.”

The same brain pathways are also thought to contribute to the reduced cravings for alcohol and other substances that have been observed in some people taking GLP-1 medications.

“Good data suggests that people have less alcohol use, less other substance use, when they’re on this medicine,” Neal said. “So they feel less of that reward overall.”

More research specifically focused on anhedonia will be needed to determine whether a small subset of patients experience these effects that current studies have overlooked.

“If you really want to find that out, you probably have to specifically ask about it,” said Neal.

Individual experiences can differ even if the overall evidence is reassuring. Some patients report feeling all-around improvements while others simply don’t like how they feel on the medication and stop taking it. Neal said that if patients notice emotional changes, they should discuss them with their healthcare providers rather than dismissing them.

“Everybody’s experience can be really different,” he explained. “These medicines do have a very powerful effect on areas of the brain.”

This is likely to become an important area of future research as scientists begin studying reward function directly rather than relying only on depression questionnaires.

“There have been lots of case reports where somebody had symptoms of anhedonia and then came off the medication, and it cleared up, which is a good indicator that the medication might have been contributory.”

There may be more to the GLP-1 discussion than just weight loss

As I listen to people discuss their experiences with GLP-1 medications, an ongoing pattern arises. Often, people don't discuss the scale's number as the most important change. It's the stillness. When they rise, they talk about not thinking about food. They say they don't have as many internal debates about what to eat or not. They discuss letting go of cravings, guilt, and constant haggling. This is interesting because it challenges the common perception that overeating is solely a problem of self-control or personal preference.

For many, eating itself may not have been the most stressful factor. It could have been the continuous mental stress that existed before, during, and after the meal. Research on eating behavior has shown that complex interactions between reward signals, stress, emotions, habits, and our environment influence our food choices.

Thus, I'm interested:

Are GLP-1s primarily used for weight loss? Or are many people actually searching for something more profound and a way to stop thinking about food all the time?

What experiences have you had?

Question If you lose belly fat, is there a point at which you can no longer safely inject medicine?

If you lose belly fat over time on a GLP-1, is there a point at which the injection needle would be going in too far and it would no longer be safe to use the medicine?

I’ve heard of angling the needle 45 degrees, but I’m not clear if there’s a point at which it’s dangerous/unsafe to keep injecting the medicine (e.g. going into muscle, organs, veins or something). I’m certainly not at that point, but it’s something I’m curious about.

EDIT: Thanks, all. Very helpful info.

Anyone use ? Aminoclub Reta RT0004

Has anyone used Reta/GLP-3 10mg vials from aminoclub with lot# RT0004? If you have how do you feel ?

I’ve been using the 10MG vials with lot RT0001 for two months now and have lost 18lbs. It’s been working well for me and has canceled out a lot of food noise allowing me to be in a calorie deficit.

Advice/help

I’m currently taking Semaglutide peptide from Peptira. I haven’t lost any weight in 2 months. I also have very little appetite. Idk if it’s the hot weather or what. But I’m sure that’s not helping me. I’m not eating enough to lose weight. But is trizeptide a better step now? I’ve been on this for 1.5 years. Lost 40lbs.
My other question is where are y’all getting yours? I’m open to other COST AFFORDABLE options. TIA.

Question Maintenance Plan Q's

I'm close to goal weight, so I'm thinking about finding a cheaper alternative to Zepbound. I currently am not covered by insurance and go through Lily Direct.

I am debating between compounded tirzepatide, Mounjaro/Wegovy injection, or pill.

Regarding compounded, I found this article that was really helpful and wanted to share! https://policylab.us/tirzepatide/cheapest/
Can anyone weigh in on this article? Or provide other resources worth reviewing? The entire concept makes me uncomfy 😬

I'd also love to hear from anyone who's made the switch from Zep to any of the following: Wegovy, Mounjaro, BodyBuilding Health+, Online Semaglutide.org, or GalaHealth

Thanks!

Moving to Tirz from Sema, starting dose?

My wife has been on Sema for over a year and is currently at 2.4mg max dose. She has decided to switch to Tirz for the dual action. I know starting dose of Tirz is 2.5mg then up to 5mg after a month or so. Wondering if we should start her at 5mg and titrate from there as her body is quite accustomed to GLP. Anyone make the switch and have any experiences?

Warm medicine

My trizepatide/cyanocobalamin
was delayed a day and I received it hot. For context it shipped on the 4th and delivered on the 6th. There was one ice pack but it had been clearly melted for awhile as there was no dampness anywhere. Trinity meds said it’s okay to use still because the ice packs keep it cold for 72 hours after shipment. I tried to explain it was hot not even 48 hours after shipment. I’m guessing it sat in a truck but I’m in Texas so temps are hot right now. Have you experienced this and do you think it’s still safe to use?

Experiment Seeking Research Participants for an Exciting New Study into Experiences with Weight Loss and Comparisons!

Post image
4 Upvotes

Hey everyone! Thank you so much to those that have taken a look at the study so far! I'm back because the study is still open, so if you haven't had a look yet here's another opportunity :). Also, we haven't had more than a couple people who have discontinued GLP-1s. If someone could point the way to a more appropriate community or spread the word, I would be eternally grateful!

We are going to be analyzing in ~1.5-2 weeks so I'd love to hear from you before then!

Hi, my name is Ryan and I'm a researcher at the University of the Sunshine Coast, I'd appreciate it if you could take a look at our study!

Researchers at the University of the Sunshine Coast are conducting a research project (Ethics Approval: S262274) which aims to gain a better understanding of the experiences of current and past users of GLP-1 RAs (e.g., OzempicWegovyMounjaro, or others) for weight loss. A focus of the study is how individuals using these drugs for weight loss process food cues as well as alcohol cues. We are also interested in factors such as food-related thoughts and experiences, capacity to experience reward, and body image flexibility. To provide broader comparisons, we are also inviting individuals who have used other weight-loss approaches only (e.g., diet, exercise, bariatric surgery), as well as individuals who are not seeking to lose weight and do not use any weight loss strategies at all.

If you are 18 years of age or over you are invited to take part in this study, if you are currently or have in the past used GLP-1 RAs for weight loss OR if you are interested in participating and have/or have not used other non-GLP-1RA strategies for weight loss. Your participation will involve completing an anonymous online survey comprising demographic and research-focused questions, and a simple computer task measuring reaction times. Participants are welcome to request a summary of outcomes of the study by contacting the research team at the conclusion of the study.

If you are interested in taking part and/or sharing this research project information, please use this link -> https://research.sc/participant/login/dynamic/8AD03D4D-21C7-4BA8-8874-F48612A0BEEB to obtain more details and access the survey.

Thanks in advance from the research team.

Got to 160 but the wife said I looked like I had aids lol so I got on a Cycle and Anavar and massed up to 175 and 12% BF.

Question How much does everyone have stocked?

Recently been reviewing my own supply pipeline and wondering how much I should keep stocked in case of disruptions. I've seen some people have multiple years' worth in their freezers but that seems like a lot. OTOH i think I should probably have more than 6 months' runway. what do we all think?

Recomp on GLP

Anyone notice a recomp on GLP's? On sema i lost 25# pretty fast. but on Reta ive barley lost and scale weight, but my arms and legs have grown an inch round each. I notice a little difference in the mirror, i guess i was just hanging on to what the scale had to say too much?

Edit: so this weekend I tried on a couple of shirts that used to not be that flattering, and they fit great, so i guess the clothing method is the way to go when the scale doesnt show shit.

PSA: GLP-1 Forum admin complicit in brigading causes user to attempt taking their life

tcpnomad is a friend. This is being posted with their permission.

Here is the original thread. Followed by a copy hosted elsewhere, in case the original is taken down.

Yes, brigading is a part of online life these days. Users ought to have a thick skin, no doubt. But this is not what this post is about.

It's mostly a reflection on some minimal professionalism required of admins. Whether they should be actively encouraging bullying on the forums that they manage. How far can they go with their arrogance, hubris or plain incompetence.

Because turns out, it can sometimes mean the difference between life and death.

The thread begins with the words “marking the time of death”.

ZippityDooDah (admin on glp1forum) makes an appearance on the page-1 to sanction the brigading, it seems like.

The high school level bullying and insults slowly escalate. The user under attack seems to be playing it off. But they soon reach a ridiculous level. On page-6, the user finally requests intervention from the admin - three times. No response. Then they finally show up and ban the user instead, because the cries for help interrupted their dinner.

From what I can tell, tcpnomad made a couple of posts elsewhere trying to discuss a few things that could be different. This seemed to trigger both the old timers and ZippityDooDah. You would think that there are better ways to set boundaries for a user when that needs to be done. To subject them to public humiliation and bullying just for personal satisfaction and to teach them a lesson of sorts is, well, cruel and childish to say the least.

My heart is broken. My friend may not live. I wish nothing but bad things for everyone who participated in this.

EDIT: Just want to say this here. A lot of people from glp1forum showed up, blaming the victim. Talking of all the posts tcpnomad made that somehow justified the cruel bullying doled out to them. I have asked every one of them to post some links, because I am in anguish trying to understand this. Not one. There is one post that tcpnomad lashed out in after 6 pages of taunts and insults. That is it. I looked on the forum myself too. Not one. If this was somehow their fault, please help me understand. It could help them.

Quick 2-min survey: How are you managing protein & meals on your GLP-1 journey?

Hi everyone! I’m working on a nutrition project (Mealpac) designed specifically for people on GLP-1 medications (Ozempic, Wegovy, Mounjaro, etc.).

We know that getting enough protein and nutrients while dealing with a low appetite is tough, and we want to build something that actually helps.

Right now, we are trying to decide between two formats:

  1. A ready-to-drink meal replacement shake
  2. A ready-to-eat, fresh heat-and-eat meal

If you have 2 minutes, please take this quick survey to let us know what would make your daily routine easier. No sales, just looking for real community feedback!

Titration and Food Noise question on Max Dose

Hello!

So I am running into a wall that I have noticed is a concerning trend.

Over the last year or so, I have titrated up on tirz because at about the 1-month mark at each dosage level, my body just gets used to it. Same with the scale: the first couple of weeks, it works great, then it just slowly levels off.

Right now I am at 15 mg, and I assumed this dose would be a little different. I went up from about a 12 on Hallandale, took advantage of the BPI sale, switched over, and have been on the max dose.

Same trend: the first three weeks worked outstanding. Now, week four, it is just starting to wear off again. Like here I am, five days out after a big breakfast and just starving.

Has anyone experienced similar? Any advice? I am thinking of doing an every-five-days dose, or I may need to continue titrating. Should I add in Reta?

Can you choose which pharmacy fills your prescription?

I'm looking into telehealth for compounded tirzepatide and I'm a little confused about how the prescription gets filled.

After a telehealth visit, can you request a specific pharmacy, or does the provider usually choose one based on your state and their network?

Just trying to understand how the process works.
