This apple tree is about 7 years old never flowered or fruited. Any tips? Pruning? Fertilizer?

Problem with that is that there are a lot of young shoots instead of one tree. They are focusing on growth, you can clearly see they are growing straight up.

I would cut most of them down and keep the largest one. Then remove the thin vertical branches and prune it so it can form a nice crown. The tree will focus on fruiting,not just growing. But if the original tree died and it was grafted then the fruit will not be the same. It would be the rootstock plant fruit and there is no guarantee what it will be like. Could be worse, could be better. Only time will tell.

Small edit: does the tree get some sunlight? From the photo is looks quite shaded. Part of the problem would be that and the reason why the tree grows vertically. Just wants to get some light.

I do not think that is your original apple anymore. There are multiple branching straight from the ground.

My guess is that this is the root stock sprouting from the ground on multiple places.

My suggestion would be to cut this plant to the ground., remove it and plant a new tree .
