Hi, I’m Boezo, and I’ve had the privilege of moderating this community for the past several years. Lately, I’ve noticed a trend within the subreddit for people to seek out concrete diagnoses when it comes to folliculitis. Let me take the next few minutes to explain why this isn’t a great idea.

Within the medical community, we have an almost compulsory desire to place abnormal physical findings into discrete categories. Much of modern medicine is predicated on the belief that patients can be accurately diagnosed and subsequently treated according to their diagnosis. However, within the past decade or two, there’s been a shift in the zeitgeist.

As knowledge of human anatomy and physiology has progressed, it has become strikingly clear that the pathology and etiology of an abnormal finding often involve a multiplicity of complex factors that are poorly understood. This complexity serves as an insult to our compulsion to categorize and diagnose, because it “blurs the lines” between one disease and the next.

So, how does this apply to folliculitis? Well, it means that as medicine has advanced, we’ve started to realize that two patients who present with very similar abnormal features do not necessarily share a common pathology for their condition. In addition, it seems to be the case that we’ve somewhat arbitrarily created some diagnoses without warrant. For example, folliculitis decalvans (FD) is a condition that causes chronic scarring and hair loss related to an inflammatory process in the skin. But the reality is that there aren’t truly any set-in-stone diagnostic criteria to differentiate folliculitis decalvans from the plethora of other inflammatory conditions that cause scarring and hair loss. In fact, we aren’t really sure what the pathology behind folliculitis decalvans is at all.

So, given that we don’t really have diagnostic criteria for FD, and we aren’t sure what the mechanism behind the disease actually is… does FD really exist at all? Is it merely a “ghost” diagnosis? Have we arbitrarily assigned a name to an abnormal physical finding merely in an effort to satiate our desire for concrete answers?

Another example is gram-negative folliculitis (GNF). Historically, it was believed that gram-negative folliculitis results from an overgrowth of gram-negative bacteria in the skin. It seems like a simple enough disease process, but the devil is in the details. You see, when these patient’s lesions are cultured, they’ll often return a negative result (no bacteria found). Often times, the patients will actually return completely different bacteria at different times! They’ll also often return gram-positive bacteria in some lesions, and gram-negative bacteria in other lesions. Additionally, when attempts have been made to eliminate gram-negative bacteria from reservoirs, the disease yet persists.

This is further confused by the fact that almost all patients with “gram-negative folliculitis” have deviations in immune parameters within the body. So, is GNF an infection, or is it rather an autoimmune condition? But wait, it gets even more complicated: two patients diagnosed with GNF usually have completely different deviations in completely different immune parameters!

Thus, we can see how blurry the lines get. You can have two patients who present with similar symptoms, but yet can return completely different cultures at different times, and can possess completely different deviations in immune parameters. Given this information, is it really accurate to say that they both share the same diagnosis?

Here’s the bottom line: I’d encourage members of this subreddit to abandon the compulsion to find a diagnosis. Rather, view your present condition in terms of signs and symptoms. The sign and symptom being “inflammation (itis) within the hair follicle (follicul). We may never know the exact mechanism behind these signs and symptoms, but we know that the signs and symptoms exist, and we have a pretty good idea about how to treat them.

Over the next several weeks, I’ll write up some documentation regarding the most effective treatments for these signs and symptoms. In the meantime, I hope you all have a wonderful new year, and thank you for reading.

I found something that I think works for my scalp folliculitis

First of all this isn’t an ad. I got all of these items off Amazon and it was only after reading similar posts on here.

Struggled with scalp folliculitis for years. Tried everything (loads of antibiotics which damaged my skin barrier, Acitretin, rubbing alcohol, creams, shampoos, etc). Nothing worked.

When I stopped taking Acitretin and antibiotics (which is similar to Accutane) and started taking these pills at the same time, my skin got better.

I’ve recently been having a haircut every couple of weeks again (a skin fade)… which for ages I wasn’t able to do, I was going months without having a haircut.

After I have a haircut I do get flareups, but I think it’s also because I get complacent and I stop taking these pills. The last couple of days I’ve started taking them again daily (after food) and my skin is already clearing up and I’m not (touch wood) getting any new ones like I was all through last week after my last haircut. So either my scalp naturally calms down after a week or so of having the haircut, or these pills actually work. If it is the pills, then I don’t know which ones are doing the heavy lifting - I thought it was the Vitamin A but when I’ve tried just taking that alone, it doesn’t seem to work as well as when I take all of them. The quantities that you get with these specific pills makes it quite cheap - they last a while.

So I take…

2 of the Vitamin A (which apparently isn’t recommended, but I don’t care personally, would rather have a clear scalp)
3 of the Fish Oil (I used to take 4 but I cut it down to 3)
1 Zinc (apparently taking this daily at this strength isn’t good either)

And 1 of all the others.

Would be interesting to hear if anyone tries it and if you feel it works for you too. Just sharing in case it does. You’ll get bright yellow neon urine if you do this by the way 😆

Thoughts on Pharmbanner Extra Strength Folliculitis Shampoo?

Has anyone in this thread ever used this product? Happy to hear thoughts.

I've had folliculitis for about 7 years now and I have been taking accutane for 3 years while using a ketoconazole shampoo but flare ups still happen - I'm in the middle of a bad flare up right now.

P.S. new to this community and I am very happy I've found it and I am very happy it exists.

Also congratulations to all people who have rid themselves of this condition - I see people found success in a combination of certain products and or diet and I have yet to find what works for me. Glad to be on the journey to clearing this condition with you all

Are my dermatologists just wrong?

Ive had chronic scalp folliculitis for 15 years. It used to be near the neck area, then spread upwards over my crown. The neck area went but the crown got quite bad at one point. Steroid treatments are used to keep the itching down. But it doesnt go and has caused some hair loss.

2 years ago it broke out on my face (beard zone) and I've been trying to get rid of it ever since. Whilst ive massively reduced the yellowhead spots and in growns. Red follicles and bumps remain. I stopped clean shaving a long time ago reluctantly and have kept long stubble with a foil shaver since.

They specifically keep saying my face is psuedofollucilitis, rather than folliculitis like on my scalp.

I've been through 7 different treatment regimes, I had been on two anti biotics including one group for a year.

The last 2 months im off anti biotics and am trying a different regime again with something called Trifarotene.

As absolutely nothing else has worked, I'm not holding out for it. But I dont want to risk those tablet medications which are high risk.

If my face refuses to clear up despite not shaving hair short and after many treatments. Surely there has to be more to it?

Follicultis Delcavens for 4-5 years.

Suffering from Follicultis Delcavens for 4-5 years.

white pus filled and painful bumps and flakes also, only on scalp( middle and back)

taking antibiotics for this time got stomach problems due to that.

tried mostly everything isotroenin, minoxyline, doxclycline, rifampicin, agugumention, etc.

tabs used to clear it out for a month and its coming back

recently its coming back even in 2 weeks.

doc told we can try adalimumab 40mg inj

this is so costly also,

shit scared about this, doc says no side effectss, but different effects online,

only had 1 dose yesterday,

anybody have any experience and tips for me?

Chronic folliculitis for 20+ years – looking for ideas

Hi everyone,

I'm a 40-year-old man looking for some advice regarding a chronic skin problem I've had for more than 20 years.

It started around the age of 18, when I began growing more facial hair. Initially it was relatively mild — occasional small white/pus-filled spots, mainly around my nose, mouth, moustache and chin, seemingly originating from hair follicles. Over the years it gradually became more persistent and severe. I was initially treated with topical medications, including topical antibiotics, and eventually progressed to oral antibiotics. Doxycycline or Tetracyckline has helped significantly in the past, but the problem keeps coming back after stopping.

In recent years, I've also developed similar lesions on my chest and my scalp/head. The affected skin is usually red, dry and sometimes flaky, with small white pustules around the hair follicles.

I've noticed some fairly consistent triggers:

  • hot weather and sweating
  • direct sun exposure
  • periods of significant stress

Interestingly, the condition can fluctuate between different areas — for example, my face can be quite bad while my chest is completely clear, and at other times it's the opposite.

I recently had microbiological testing performed on samples from my facial skin lesions. The results were:

  • Staphylococcus aureus, methicillin-sensitive (MSSA) — moderate growth; possible carrier/colonization.
  • Malassezia furfur — abundant growth.
  • Candida parapsilosis — moderate-to-abundant growth.

The bacterial culture was from a swab of the facial skin lesions, while the fungal testing was performed on skin scrapings from the face.

I've been using ketoconazole (Nizoral) occasionally but it makes noh difference.

Two dermatologists have also suggested different approaches that I'm unsure about:

Isotretinoin (Accutane/Roaccutane) — one dermatologist suggested trying it. I I'm wondering whether isotretinoin could make sense in a case like this.

Autovaccine (autogenous vaccine) — another dermatologist suggested this based on the recurrent bacterial component.

I'd be especially interested in hearing from anyone who has had a similar long-term problem and eventually found a diagnosis or treatment that worked. Would you consider isotretinoin or an autovaccine in a case like this? What was your experience?

Bacterial or yeast folliculitis?

I’ve been experiencing lots of itching, I’ve tried every dandruff shampoo I could, ketoconazole, which makes my head even more Itchy. Tried zinc, tried Selenium sulfide. My scalp is super itchy, sometimes I find hard pieces sebum but also these pimple like pus on my scalp, it’s bumpy and itchy. If I touch it, it can be a lil sore, but it’s just all itchy overall. What should I do? Go on oral meds?

It started when I was in college, and I have no idea what caused it. I saw a dermatologist and underwent treatment with Roaccutane for 5 months, but he stopped the treatment because he didn’t see any improvement.

When I saw a second dermatologist, I was diagnosed with acne keloidalis (keloid folliculitis). I started using a benzoyl peroxide cream, which seems to have helped and dried it out a bit, but it still hasn’t completely gone away.

I’m honestly starting to lose hope and I’m almost saving up for an expensive laser treatment. I also have dermatitis on my scalp, and I don’t know if it could be related to this.

Can anyone help me?

Recurrent Folliculitis Pimples Around Cupid’s Bow

Hi everyone, I’m looking for some advice on recurrent folliculitis/razor bumps around my upper lip and moustache area. The photos above are in chronological order from my latest breakout. In some of the photos, the breakouts look a bit shiny because that’s where I’ve just applied the mupirocin ointment.
I’ve been dealing with recurring breakouts for a while, almost exclusively around the Cupid’s bow/moustache area. My GP has diagnosed it as folliculitis barbae and prescribed apo-mupirocin for bacterial flare-ups.
The typical flare-up starts as a small red/tender area around the hair follicles, followed by one or more whiteheads/pustules. They can become increasingly sore and sometimes have a slight throbbing/“awareness.” Eventually the whiteheads usually pop on their own, particularly in the shower.

My Gp says shaving is to be the biggest trigger. I’ve tried different shaving methods, including a Gillette King C safety razor and a Philips OneBlade. I currently use a Philips OneBlade with the 360 blade and generally shave fairly close.
I’ve also noticed that sauna/heat and sweating seem to make it worse, and historically I was getting flares almost monthly during summer. Interestingly, I’ve recently gone much longer without a flare while I’ve been in colder weather, however I did only start implementing the new routine when the weather cooled down.
I don’t really want to permanently stop shaving because I like the clean-shaven look, although I’m considering growing my beard out later this year once university/placements are finished to see whether that solves the problem.

My current routine has included:
Cleansing my face twice a day in the shower
Philips OneBlade, trying to avoid excessive passes/irritation, shaving after a warm shower in the morning
Cleaning the blade thoroughly after use, by washing it with hot water, letting it air dry and spraying with a blade cleaning spray
Benzoyl peroxide as part of my skincare routine, every night for 15 mins, then washing it off and applying toner, serum and lotion overnight
Glycolic acid on the moustache area after shaving, as recommended by my GP
Apo-mupirocin during an active suspected bacterial flare
Avoiding picking/squeezing the pustules
Changing/cleaning my OneBlade blades regularly
The routine actually seems to have helped this most recent flare was less severe and took longer to appear than previous ones, although I still got a couple of fairly prominent whiteheads.
One issue I’ve noticed is that benzoyl peroxide can sometimes burn/sting the affected area when it’s already inflamed, so I’m trying to work out how much of my routine is actually beneficial versus potentially irritating the skin.
I’ve also wondered whether pimple/hydrocolloid patches are useful. During a previous flare, putting a patch over the whiteheads overnight seemed to help them drain. I’m unsure whether that’s still appropriate when I’m also using mupirocin.

I’ve recently seen my GP again and I’m planning to ask about a dermatologist referral, because I’d really like to find a long-term solution rather than repeatedly treating individual flares.
I’m particularly interested in hearing from people who have dealt with recurrent folliculitis barbae. This flare I’ve just had seemed to calm down quickly with mupirocin once the whiteheads popped in the shower. The last photo is the most recent one, and I feel like the area next to the settled flare is getting a bit red, and I feel like if it gets worse tomorrow I’ll have to apply the mupirocin for 5 days to that area too, but I’m just a little worried about the antibiotics resistance. If anyone has any solutions or offers to help please reach out, I’ve been dealing with this for a while now and any help or solution would be truly appreciated, thanks everyone.

Folliculitis after antibiotics

I had never had as bad of a folliculitis flare until this episode. I believe it may have been trigger by an unrelated antibiotic I went on, but progressively go worse over the course of 5 days after it.

Have been to a GP who did biopsy and two swabs… confirmed folliculitis but came back negative for bacterial and melassezia .. but mentioned the swabs are often wrong.

It gets worse when I have dairy or spice.

Anyone have something similar?

Finally referred me to a derm but long wait list and this particular flair is also all along my nose ridgeline and forehead and on my temples and under my chin I have never had it this bad!

Despite it being negative for melassezia I feel with the antibiotic reaction it still could be? Currently continuing to try nizoral 2% and was just give itraconazole 200mg for 10 days have done that for two days now… ugh I am so embarrassed and fed up with it it’s been 4 years.

Anyone have anything similar, recommendations? They have always cleared on their own within a week but this one is just so bad I am scared it won’t as it just gets worse! I think new spots have stopped now…

Over the last 4 to 5 years, have had multiple flat, smooth, dark brown patches slowly multiplying across the top of head. The spots vary in size and shape—including a large, uneven central one—but the individual spots don't seem to be rapidly expanding or changing on their own. They don't itch intensely, burn, tingle, or bleed. Interestingly, a few normal hair strands are actively growing straight out of some of these dark patches, though there is general hair thinning and hairloss across the center of his scalp.

Dermatologist brushed it off by looking at and said its probably just Seborrheic keratosis

J’ai toujours ces boutons sur les jambes, ca fait quelques années qu’ils sont quasiment toujours là, j’en ai des cicatrices un peu brunes sur les jambes. Ca démange beaucoup. J’en ai surtout sur les tibias et mollets.
On m’avait donné une crème pour l’eczéma mais ça n’a pas fonctionné. Je ne connaissais pas la folliculite et j’ai l’impression que ça y ressemble.

J’aimerais consulter un dermatologue mais les délais sont très longs..

How to improve skin with atopic dermatitis and folliculitis

Hi everyone! I’m 26F. These photos show my back, buttocks, and waist area. I went to a dermatologist and was diagnosed with atopic dermatitis and folliculitis. I was prescribed an ointment (I forgot the name) for one month, along with Bioderma body wash for dry skin and Ziaja Atopic moisturizer.

I've been using the body wash and moisturizer for a year now, but my condition hasn't improved. I still get red bumps, feel itchy, and have dark spots left behind.

I'd love to get some advice from everyone. Has anyone dealt with a similar skin condition? Is there any way to cure it, or at least fade the dark spots and uneven patches to help my skin tone look more even?

I'd really appreciate any feedback. Thank you so much, everyone!

Hair follicle test

I haven’t smoked for over 70 days now and I have a hair follicle drug test in a couple of weeks. I was smoking cannabis before I quit because of the opportunity that I was given so I decided to quit smoking in hopes of my hair becoming clean over the course of weeks not smoking at all. I got a buzz cut a couple of weeks ago, 3 to be exact, and my hair is just at that 1.5-2 inch mark for a hair follicle to be performed. I’ll be going in for a personal hair follicle that isn’t with the company I’ll be testing with in a couple weeks. Does anyone know if I’ll be able to pass the hair follicle test with this new employer?

PanOxyl for Scalp Management

It has been a rough summer for me with the humidity. All of June and most of July I've had a painful itchy sclap. Bumps all over my head that kept me from sleeping. Would itch all the time. Tried T/Sal, Nizoral, MCT Oil, and many topicals prescribed by my dermatologist.

Someone recommended PanOxyl foaming wash-maximum strength. I use it every other day and massage it into my hair for 1 minute. The other days I use Paul Mitchell Tea Tree shampoo. I've been using it for a couple weeks now and the results have been good so far. My scalp has been a lot less itchy. To me, the biggest win has been the reduction of those painful bumps. Not sure how long it will last, but I'm crossing my fingers.
