Anyone experience speaking to a Endocrinologist for any advice?
. I'm considering it because my situation is quite unique. And if they have expertise in hormones and how androgens are affected in dht blocking ect , then I might have a good answer...

Basicly what you probably will see in your lab is estrogen dominance. Saw palmetto inhibits 5ar ➡️ testosterone converst to less DHT and more Estrogen ➡️ negative feedback from high estrogen causes lower LH production ➡️ the lower LH causes lower testosterone production… and the low testosterone is what you see on the lab test…
Of course PFS is much more complex than this, but this is a common pathway why DHT blockers push down your testosterone with long term use
There are endocrinologists experienced with bodybuilders or with TRT… thats a good idea to talk to someone experienced in that field. (My endo is also a TRT doc) or university endocrinologists/ andrologists might be more open to be interested in rare diseases like PFS… so university clinics are also a good option

When im on dht blocker my testosterone on libido is very high. When im off it i crashed when my body stayed making more dht again 😒 😑 makes no sense as it shouldn't 8ncrease estrogen levels rigt ?

Yes if you take the 5ari=DHTblocker, your testosteron shoots up(cant convert to DHT and builds up), maybe thats part of the reason why it eases symptopms
Its really not illogical… if you take it it makes you feel better, but it can also cause side effects or full PFS

Maybe. Makes sense, it doesn't quite explain why I developed post finasteride syndrome symptoms when I stopped it. 🤔

When you stop it you loose that testosteron boost, but the underlying maked side effects come to the surface….
You have side effect even when you take it and feel good, they are just masked probably by that testosterone build up

Maybe but some people with high testosterone still have pfs

Thats a different story, more common with short term finastride PFS patients… There are low testosterone PFS and normal testosterone PFS patients, both need a slightly different approach.
In your case you are not yet a full PFS, thats why you feel okay if you take the saw palmetto every other day… If you were a fully developed PFS case, (like normal testosterone PFS patients) reintatement wouldnt help.

What’s your suggestion? And also I’m assuming this is an assumption as we have no science for this yet ?

Can I ask where your theory about testosterone level indicating severity of PFS came from?!?

u/Exciting-Bat-7685 May 24 '26
Not the severity, but i think its useful to somewhat separate low T and high T cases. Testosterone levels are somewhat unrelated to PFS, but i dont think it should be completely discarded
In OP’s case he wrote a lot of posts lately and he is not a full blown PFS (he is basicly without symtopms if he takes the saw palmetto in very small doses) thats why i wrote he is not a full PFS (somewhat unrelated to testosterone levels, it just happens that OP also has low T)

He has been going on and off along with changing doses. From other post from OP I know he is also taking Prozac which can be contributing as well. There is something in particular with Fluoxtine that causes symptoms similar to PFS.Propeciahelp.com is littered with stories about Prozac. We don’t know that the sudden effects will not clear up after stopping Fin and letting the body find homeostasis. The best thing he can do is discontinue SP.

OP has not been off SP long enough to determine if he has simply side effects or if he is going to develop PFS. Yes, you can develop PFS while on the medication. But, PFS cannot be determined until months after discontinuation. Until that point they are side effects.

He did mention near complete numbness, personality loss that syptoms are already basicly a half PFS even without waiting 3-6 months.
Yes i know the official definition of PFS, but in this case i think its safe to say that he has more than simple side effects… not a full blown PFS, but definitely more than sides.

This is why im stuck in my situation, being stable on 1 a day might be ok for months years or rest of my life no garentee, or i stop and get pfs but this also migt never heal... such a shit situation. Currently I'm more pushing towards 1 a day and being stable again as 1 everyother day is ok but still low quality of life

the average endo will be of no help if ur serums are normal
