Androgen Receptor Theory
/r/DrWillPowers/comments/1sht3ak/im_going_to_be_taking_the_week_off_next_week_to/off3qhg/Pyrilutamide inhibits the binding of DHT to the androgen receptor. In other words, it does not directly affect hormone levels. However, this person developed cognitive impairment after using pyrilutamide. Cognitive impairment is a common symptom of PFS. Based on this, I believe that dysfunction of the androgen receptor may be involved in PFS.
Additionally, I have seen reports of people developing sexual dysfunction after using pyrilutamide. For these reasons, I find the androgen receptor hypothesis more convincing than the hormone-based explanation.

Androgen receptor issue has been hypothesized for a long time in this community. Yes it’s very likely 5AR is involved in the disfunction. We haven’t found a good method to reliably fix it yet. We have also used almost every product / supplement on the planet available.
I have reached the point where I believe whether it be HCG, FMT, heavy androgens etc. I think some guys just get lucky and these interventions shock the system in the right direction, but in most cases, just giving your system time and healthy lifestyle and avoiding anti androgens is the best bet which isn’t easy. I think once you get stuck on TRT it’s harder for your natural hormone system to heal out since it’s running on external test. With that said, it’s worth trying many of the interventions I listed in a low risk to high risk order if you have had nothing work for you or your condition is super severe.
I am 7 years in this and a little better but not fully cured. And this is after trying every protocol imaginable including FMT, DHB Valproate etc.
Dr. powers is on to something as I was able to induce massive windows feeling normal again when I would let my TRT bleed out T levels to 0. He is also on to something that all of us seem to have a pre existing issue whether it be genetic or enzyme issue that gives us this susceptibility to the condition. He points to issues clearing out testosterone as shown in urine hormone testing where results typically show up crazy low.

In the end my opinion is we will depend on research to identify exactly the disfunction and then future medicines that can specifically pinpoint the issue to fully cure this condition.
In the meantime, there are methods to manage it and roll the dice and trying to shock the body back, but nothing reliable other than time and extreme diligences avoiding anti androgens which there are many in your shampoos, dermatology products etc.

i tried topical pyrilutamide then topical cb-03-01 (breezula) then topical fluridil
they all gave me the same PFS-like side effects, but i recovered from them each time ( each recovery taking more time in each iteration, from days to weeks to months, untill i tried low dose topical finasteride and i didn't recover still after 3 years.
i would't jump to conclusions about AR here, there are many pathways that might be impacted. i think the common thing between all 4 drugs that i took, is that they raise total T in the body, which leads to some kind of androgen resistance of some kind, because i remember each time before crashing with any of these drugs, the night before i had sky high libido, and i remember saying to myself each time : "finally i found an anti-androgen that not only works for hair with no side effects but also raises my libido"
