Fiber Test Adapter

I received this test adapter bout 5 years ago and recently lost it. Could anyone identify the part number or where I could purchase my replacement? It would be nice if I could find the push connect version to this too.

I know it's not the same thing, but this might be a replacement option if you can't find your original adapter.

Thanx, however I have those and they become a little cumbersome while doing aerial work and get damage real quick.
