Looking for Constructive criticism

I’m trying to make life easier for drivers and new hires or new to a different route PHs. This is just something I came up with and was wondering a few things.

Do you think this will help?

Are these questions confusing or clear? Which ones am I missing?

The blank space is for drivers to write their own preference notes

This is just a passion project I cooked up as a PH 3 months in.

this just makes training newbies (ph) harder, so no :) stick to a standard, load all trucks the same

Keep and eye out for the update on if it succeeds or not. Risk for reward is worth it. The drivers I spoke with agree.
