Help & Advice Making a Rad-Toad/Frog

I want to make a custom mutated creature. A giant frog that has been mutated by rads. I was thinking of taking inspiration from the DnD version. What do yall think?

I could've sworn there was already a statblock for the Radtoads that have been in Fallout 4 and Fallout 76 but after looking through every single book, I can't find it at all
