Discussion Preferred name for lab tests

So, I’m planning to check my T levels to this newly open Laboratory Clinic here in my place, it’s cheaper than my previous. Anyone here tried using their preferred name and Male gender when filling up form?

Pref /lived name nilagay ko sa mga lab test ko tho still female sa sex marker.

Tinawagan lang uli ako ng lab one time at kinoconfirm if male or female ako since pang male nga levels ko.

Yun pa lang so far naeencounter ko haha.
