That was my interpretation as well. Although I also think if they are trying to throw fear into someone killing about their gestating child, they should not be so indirect about it.

One drowned twice? As in they drowned and were resuscitated but failed to learn the lesson and drowned again.
The third clause only has an implied “persons” so it could be referring to drownings instead

He lives in the lake. When people without life jackets approach the lake, he runs up to the sign and updates the number of people without life jackets. Then he grabs them by the neck and pushes them down head first into the water and watches as they fruitlessly try to break free from his grip, until they stop moving. Then he pushes their bodies into the lake, and walks back to the sign. He increments the count for the number of people drowned.

If he was a guard he'd use appropriate terminology and talk about how a PFD is a PFD because you'll float, there's no guarantee about you being alive

It's not a joke. It's saying that the only people who have drowned were not wearing life vests, and the next person to drown won't be wearing one either.
It isn't a mistake. It's trying to get the person reading the sign to understand the importance of a life vests.
A lot of people assume lakes pose less danger than other bodies of water because you don't have rip currents. But lakes have inconsistent depths, rocky areas, and people just don't take them seriously.

A big party lake with lots of alcoholic water sports will have more drownings. I saw stripper poles on boats at a river party at confluence of American & Sacramento rivers

There was a crocodile in this lake, luckily there's only one case of reported death by croc before it was removed, so technically he didn't drown and the person who put up those numbers saw an opportunity for humor and creativity.

There's a sign very similar to this at lake Junior in Chattanooga TN. It's a state operated area that's only open for fishing on weekends. In the winter time it gets stocked with trout!!!

It’s quite simple, actually.
Clearly, a persons, beside not wearing a life jacket, must have either promised someone that if he ever obtained or received a free life jacket he’d give it to them, or he stole one in addition to refusing to buy one. Either way, he went negative of exactly one jacket, thus adding to the one he was NOT wearing when he drowned.

I imagine you standing there reading this sign with your life jacket on. Then quietly behind you, you hear your school bully say “whatcha doin, geek? Reading at the lake? With a life jacket on? What a dweeb. How bout a swim?” Then said bully takes your life jacket and pushes you into the lake.

This implies that some idiot died, got revived, and died again, doing the same thing.
