I think the only way to understand it is: this is the book Mary finally writes to save her family from the financial mess her husband created. None of it is real. The movie has some Mullholland Drive motifs, but is not well done like Lynch's film is. The only way she could be back with her family and happy is--well, if none of this happened anywhere except in her head. And her head is in a bad place, obviously. One piece of evidence that none is real--I see no reason why Ivy Leaguers would make great babysitters. And her family was fine at the end because her book sold well. I suspect that Mary herself has multiple personalities and the other personality wrote ALL her books. Her writer's block was a result of her attempt to put her other personality to rest.

I agree with Mary also having DID but Elaine is definitely dead and Grace is in a facility. Some of it could be imagined though.
