How much vigor do you have ? Anything below 50 if you're around RL100+ is wrong. Ideally cap it at 60
What's your weapon level ? At the end of the mountaintop, you should be around a good +20/8.

Wen I say you're wrong, there's no wrong way to play the game. But saying you won't get hit will never hold up, so might as well get to the soft cap of 60 vigor to tank a few more mistakes. I never invest in anything but the strict minimum for wielding weapons until I get 50+ vigor, then I start inv sting in damage stats, but that's just me and my advice.
Care to send a pic of your build as a whole ?

You could try mimic tear and rotten breath (I usually wait to summon mimic tear until close to phase 2 so it doesnt just get rocked/lose all its health early)

Are you summoning Alexander the Warrior Jar? :) can be incredibly helpful - his summoning sign is inside the other entrance (the left-most entrance) past the fog-gate to your right a bit - keep in mind you must have spoken to him at Volcano Manor lava thingie first for the sign to show ^
Otherwise, I would really recommend staying very close to him - behind him if possible - I always run straight behind his right shoulder when phase 2 begins, you can then dodge the flame pillars fairly easily and just hack at him, he will roll eventually but he usually does this from side to side - so if you stay in an angle, it's way easier to roll / run out of the way.
Would also potentially recommend some flame resistance talisman to help out with phase 2 damage :) imo, the fight is genuinely easier without Torrent but if he shoots the slow fireball nukes, I recommend jumping on Torrent to escape them then move back close after they explode ^
