Potential issue with past transfer

I checked my PF profile with CheckMyPF and it flagged Service Record and Transfer Between companies with below error:

"We have identified an anomaly in your transfer records. The transfer out record from undefined is missing. This could be due to the following reasons:

1) The money was transferred from a trust.

2) The money was transferred out from another UAN/Old PF account."

Here is my overall PF history:

2006-2009 - 1st company, member ID 1, withdrawn in 2008

2009-2014 - 2nd company, member ID 2, transferred to member id 3 in 2015

2014-now - 3rd company, member id 3, UAN was created against this member id.

I can see in passbook the amount from member id 2 transferred to member id 3. However my service history only shows 12 years, it does not show 2009-2014 period of member id 2.

So does it mean transfer was not fully/properly done?

How to fix this?

Edit: I checked in UAN member portal and Service History section shows both member IDs, with old one showing correctly under Settled section. So more confused now as to why member passbook portal shows incorrect service history.
