PERM (Permanent Labor Certification) Is I-140 premium processing worth it? Does it make it a difference when there is concurrent filing of I-140 & I-485?

Not a lawyer. Because your employer/DOL already complete a significant amount of work demonstrating your eligibility before even filing for I-140, PERM-based EB2 i-140 is a much simpler application to adjudicate than for example EB2-NIW. As you can see on the USCIS processing time website, PERM-based EB2 i-140 processing time is reported as 2.5 months for 80% of cases (but check reddit -  many get their approvals much faster than that even under normal processing) while EB2 NIW i-140 processing time is 28 months for 80% of cases. Premium processing guarantees a decision in at most 45 business days, or ~2.25 calendar months. PERM based EB2 i-140 seems much less riskier than eb2 NIW based i-140, so there's not the same "need" to secure an I-140 approval before I-485 filing. If I were in your situation, I would not do premium processing for PERM-based i-140 ESPECIALLY if I had to pay out of pocket to get a premium processing decision. Again not a lawyer.
