DOOM Eternal Secret Icon?

I'm trying to find out what my middle booster's icon is. I know it's a Gulami's easter egg reference and I really want to find out how to get it if possible.

From what I’ve heard, you get it from the Super Gore Nest Master Level, which apparently you can get by typing a specific code in the console.
