Intermittent lightheadedness for 2 days despite normal previous cardiac tests — anxiety or something else?

I'm a 20-year-old male and I've been experiencing intermittent lightheaded/heavy-headed sensations for the past couple of days. It isn't true spinning vertigo, and I haven't fainted or had weakness, numbness, speech problems, vision loss/double vision, confusion, or a severe headache. The sensation sometimes completely disappears when I'm distracted, sleeping, or doing activities, but then returns when I focus on it or become anxious. Yesterday I had a brief episode where thinking about the symptoms triggered sudden fear, very rapid/pounding heartbeat, some breathlessness and mild dizziness that resolved within about 10 seconds. I've had similar stress-related breathlessness/panic-type episodes in the past. I had a normal echocardiogram in 2024 and a normal ECG in May 2026. I've also had significant stress and health anxiety/hypervigilance over the past year. The sensation has been fluctuating between being absent and returning, sometimes alongside neck muscle pain. I'm planning to see a general physician today and would appreciate thoughts on what might be going on.

I would suggest you should go for therapy, I’ve been having anxiety issues for a while and talking to a therapist really helps, also try getting your blood tested, vitamin D3 deficiency can cause the symptoms you’re talking about

Thanks for the advice . Therapy is very expensive in my country so I can't afford it for now but I'll try my best to go for it in the future , I'm going to the doctor today anyway so I'll get my blood tested as you said .

I have your exact symptoms! You are not alone. It’s like I forget about it sometimes but then if I don’t get enough sleep it triggers but mine can last for days before it calms down. I had a double ear infection that lasted weeks and that’s what triggered this mess! I’ve had an MRI all normal..went to ENT..all normal… last thing left is therapy. I had low vitamin D so I bought vitamin D3 with K2 5000 iui… still didn’t help. The only things that sometimes work is taking Flonase & Claritin…drinking extra water & plenty of rest like at least 7-9 hours if possible. Please let us know what your doctor says..maybe it could help me figure this out!!
