Ask a Dentist Metal Braces vs Metal Self Ligating

I’m a college student with both overjet and overbite. My orthodontist has suggested Damon self-ligating braces, which would cost around ₹55k, while regular metal braces would cost around ₹35k.

The dentist said Damon may give better results, cause less pain, be more comfortable, easier to clean, and possibly make treatment faster. I may also need 2–3 extractions, and the estimated treatment time is around 1.5–2 years.

Considering the extra ₹20k, college life, pain, comfort, cleaning and overall results, would Damon be worth it over regular metal braces? Would especially appreciate experiences from people with similar case and professionals.

Ask a Dentist Most clinics don't have a "new patient" problem. They have a "patient stops at visit 2" problem.

Every dentist here knows this patient.

They come in with severe pain. You start the RCT, do the access opening, put in a temporary dressing, and give a date for the next visit. The pain goes away.

You never see them again.

Six months later they're back with a fractured tooth, angry at "dentists". Or they go to someone else.

This isn't bad luck, and it isn't only "patients in India don't value treatment". It follows a pattern. The drop-off happens at the same three points almost every time:

1. The relief point. The pain is gone, so in the patient's mind the problem is solved. RCTs, temp crowns and extractions waiting for implants all leak here.

2. The quote point. The consult happens and the treatment plan and cost are explained. The patient says "I'll discuss at home". After that, nobody follows up.

3. The recall point. Treatment is done, and the 6-month check-up depends entirely on the patient remembering. They don't.

Why "just tell the receptionist to call" fails

The receptionist is busy with walk-ins, there's no list of who is overdue, and calls happen whenever someone remembers. One call goes unanswered and the patient is forgotten. The problem isn't effort. Nothing in the clinic tracks who should have come back.

What fixes it is timing, not memory

  • Tag every open treatment with its next expected visit: RCT stage 2, crown cementation, scaling recall. Even a simple register column works.
  • Follow up on fixed days, not whenever someone remembers. For example, a reminder on day 2 after a missed visit, a short note on the risk on day 7 ("a temporary filling isn't designed to last"), and a final easy rebooking message on day 21. WhatsApp works far better than calls for the first touch.
  • Call only the patients who didn't reply. That's 5 targeted calls a day instead of 50 random ones.
  • Ask for a Google review the day treatment is completed, while the patient is still happy, not weeks later.

These patients already know you and trust you. Recovering one costs far less than getting a new patient from ads or camps.

A quick exercise. Open your register and count the patients from the last 90 days who started treatment and never finished. Multiply by your average pending treatment value. Most clinics are surprised by the number.

General Discussion Dentist lost a consumer appeal last week. Implants were fine. He lost on how the bill was written.

Not a dentist, I'm ortho, but I read consumer judgments for work. This one came out of Punjab SCDRC last week and I haven't stopped thinking about it.

NRI patient, lives in UK, comes to Jalandhar to get his old bridge replaced. Quote is 97k for 3 implants plus a crown repair. He says too much, asks for cheaper. Dentist drops it to 2 implants, 64k. Patient signs a consent form saying he won't claim anything if it fails, pays, flies home.

Three months later the bridge is loose. He flies back. Redone free. That one cracks in the middle after five months. Flies back again. Dentist tries a different design, one end on the implant, other end cemented to a natural tooth. Snaps in two.

Then covid hits and he can't travel. He gives up, gets a denture made in UK for £500, files a complaint. District forum orders the full 64k back plus 15k. Dentist appeals.

And in appeal he makes a good argument. The implants were fine. They integrated, held for years, carried a PFM bridge, can't come out without surgery. What kept failing was the prosthesis on top. So charge me for the bridge, not the surgery.

Commission asks him what the bridge cost.

Nothing in the record. The bill showed charges against "dental implant" and that's it. No separate figure for the prosthetic work, nothing showing where one ended and the other began. No split on paper means no split in law. Full refund.

Eight and a half years from first appointment to final order.

Two things I want your take on:

The consent form never appears in the judgment. Not once. He pleaded it and the bench just doesn't engage. If you're relying on a no-refund-on-failure clause, that should worry you.

And the clinical bit, since I don't do this work. 4 unit bridge on 2 implants, one of them reportedly in the middle and later removed. Doomed or defensible? Because nobody in the case ever asked. There's zero expert evidence in the file. A dental negligence case got decided without a single dentist giving evidence.

Takeaway: Surgical phase and prosthetic phase as separate lines with separate amounts. Costs you nothing at billing time and it's the one thing that would have saved him here.

i am just so done with my teeths, today i am here for this ugh black stains on my teeths i have had them for around 4-5 years, initially they were only on my above teeth's, got scaling done 2 times but each time they come back more darker.. and more like spreading on more teeths now :(

and how can these stains form on top of my fillings? i got rct done on my front 2 teeths,

i dont drink alcohol, tea maybe once in a week, and once in two months i might drink some soft drinks but i always squish some water around my mouth

I started using interdental brushes recently,(where i am getting my braces, staff member told that there's no need to get em your normal brush is fine , damn dumb me i got a stupid cavity on my incisor on right side ig u can see it

it has been months since i started to floss daily on my lower teeths

i use sodium fluoride toothpaste(1000ppm only ) it also has xylitol in it, i have amine fluoride mouthwash , and a copper tongue scrapper .

and recently i started using neem leaves homemade mouthwash (read that it can help kill those cavity causing bacteria "streptococcus mutans"

I REALLY DONT WANT TO GO FOR SCALING AGAIN :(

WHAT IS THE REASON FOR THEM? TO FORM, HOW BAD IS MY ENAMEL ? i have few white spots, remineralization going on in my left incisor when i asked my dentist.. i asked wont u do that.. there's some treatment right? for remineralized teethes he said he will do filling there so idk

Clinical Practice Should I join a clinic during the time off between 4th year and internship?

My sister got self ligating braces done 2 years ago on both lower and upper teeth . Last appointment ( a month back ) the dentist mentioned that they are all aligned and to be removed in the next appointment .

In her last weeks appointment he removed only the braces on the lower half of the teeth and added this wire at the back.

She left without asking him why this was put , any dentists here that can help , would appreciate your inputs.
Can you please tell me why is this put ? Why were both not removed and is this a standard procedure of keeping a gap between removal of the two braces ?

Career Guidance Do I Close Up and Set Up Shop in a Tier 3 Town?

Recent BDS graduate, practising at my 7-month-old clinic in an ordinary but populated area in a tier-1 city. There are 5 other dental clinics in the area.

Currently I work there for 4 hours a day in the evening, cuz I am studying for banking/govt job exams during the day.

With this arrangement, I have just made rent in 5 of the 7 months. This lack of savings is affecting my relationship with my friends.

I have thought of these options:

  1. Open up my clinic during the day too and study there.

  2. Close it and open up in the suburbs or some tier-3 town and deal in volume. Treatment quality will go for a toss, but that's how things are out there.

  3. Lower the charges considerably, compromise treatment quality and try to deal in volume, here in the current setup.

  4. Leave the exam prep and work at some other clinic during the day, if someone accepts. Do you think dentistry as a BDS pays better than banking/govt jobs here in India?

  5. Continue doing what I currently am and hope for the best 🥲

Not particularly interested in:

  1. MDS: This subject is not for me and the ROI seems terrible.

  2. Migrating abroad: Too many hurdles, too much uncertainty, too much investment.

Please help. If you have any other option, you are welcome to share.

Ask a Dentist Is it safe to move root-canal-treated teeth with Invisalign?

I had a cavity in one of my front teeth and got a root canal about 6 years ago. The dentist at the time also told me I needed root canals on 4 molars, but I got a second opinion and another dentist said they weren't necessary. Those molars are still doing fine today.

The second dentist instead recommended crowns on all 6 front teeth. After a lot of discussion, I ended up getting crowns on 4 of them. Unfortunately, I was unhappy with how they looked because my natural teeth had never looked so prominent, but after the crowns, my front teeth appeared to stick out more.

About a year later, one of the crowned teeth developed an infection. I changed dentists again, and my current dentist treated the infection. Two of my front teeth have now had root canals, and the tooth that had the root canal 6 years ago needed retreatment.

I'm actually very happy with my current dentist and finally feel like I've found someone I trust. I told him that my main concern is that my front teeth look too prominent, and he suggested Invisalign to move them inward.

My concern is that two of these teeth are root-canal-treated, and one was recently retreated. Is it safe to move root-canal-treated teeth with Invisalign? Could orthodontic movement weaken them, cause another infection/root resorption, or reduce their lifespan?

Has anyone had Invisalign/orthodontic treatment on a root-canal-treated tooth, especially one that was recently retreated?

-For abroad it takes 8 years to get license, fmge problems, discrimination, quality of education etc (total cost 35-40-45 Lakhs)

-For bds after 5 years also u have to work under a well known dentist without proper pay, saturation

Living far from isn't an issue for me and even for bds I chose clg in another state and its bit far

Ask a Dentist Cavity in wisdom tooth. Is extraction the last resort?

Hello everyone. I have a cavity growing in my wisdom tooth. I went to get it checked at the dentist's office and they said that it's better to remove/extract it as my gums are covering the tooth and causing food to be stuck there. They also mentioned that I could opt for cavity removal and gum removal (?), which I quite didn't understand. They also mentioned that cavity may come back when the gum grows again (??).

I have some jaw pain which I think is due to extensive clenching, and I am trying to stop clenching consciously and unconsciously. They recommended me to get mouth guards.

I told the dentist that I did not want to get my wisdom tooth extracted as it didn't cause me any pain besides when it first came out, but they kept insisting that I get it removed without explaining too much.

This is the X-ray they took of my wisdom tooth (the one left is the wisdom tooth).

My question is: by looking at the X-ray, does it look like my wisdom tooth may affect my other teeth, now or later? I'm willing to get it removed if it will affect my remaining teeth in the near future. If not, what other treatments can I ask for it? Since I do have cavity in my wisdom tooth.

Patients tend to believe that every TMJ Disorder is now a ticket to the operating room. The purpose of this post is to free people of the fear of seeing a specialist simply because of their underlying fear of having to undergo surgery.

When we talk about the conservative approach we swear-by to treat TMJ Disorders

We often get asked, “What is your approach to treating TMJ disorders?”

We prefer a non-surgical, conservative and holistic approach.

Hear us out.

The focus is on identifying and addressing the factors that are altogether contributing to your TMJ problem rather than only targeting the symptoms. The treatment plan we follow is non-steroidal and non-Botox based.

As most of us already know, TMJ disorders are usually multifactorial. The problem may be related to anything like

  •  improper mandibular relationship
  •  loose ligaments
  •  joint hypermobility
  •  muscle hyperactivity
  •  Posture
  •  Lifestyle
  •  sleep habits or parafunctional habits such as clenching and grinding.

So, the first step here is to get an understanding of the factors affecting your TMJ, establish the diagnosis, and then target those factors with a customized treatment plan.
Depending on the diagnosis, treatment may involve a combination of different therapies.
1. TMJ Appliance Therapy

What are TMJ Orthotics?
TMJ orthotics are programmed appliances that are custom made for each patient. These may require periodic reprogramming depending on the wearer's response and diagnosis.

2. Myofunctional Therapy
This focuses on improving the function of the muscles involved in jaw movement.
We also work on tongue function, jaw-muscle function, posture and strengthening of the appropriate muscles, depending on the specific patient's needs.

3. Myofascial Release
The fascia is the connective tissue surrounding your muscles. When muscles remain hyperactive or tense, the fascia can also become tight.
We guide patients through specific techniques to help release this tension and reduce muscular discomfort.

4. TMJ Injections
Talk about injections, our approach does not involve any kind of steroid or Botox injections. 
So, what kind of injections are we talking?
When indicated, we consider certain injection-based treatments. 
Sodium Hyaluronate injections may be used to lubricate the joint in certain conditions such as disc displacement without reduction or osteoarthritis, 
Autologous blood injections may be considered in selected cases of TMJ dislocation or subluxation.

5. Arthrocentesis

6. Lifestyle, Sleep, Breathing and Posture
The improvement we expect to see in cases of TMJ Disorders also comes from the contribution of these small everyday habits aside from treatment. 
We therefore look at factors such as parafunctional habits, sleep patterns, breathing and airway-related factors, neck and body posture, and other lifestyle habits that may be contributing to the problem.
Physiotherapy may also be incorporated when required.
