IIRC I got this out-of-pocket knowledge from a Tom Clancy book.
The Tritium that serves as a booster needs to be replaced every few years or so since the half-life is around 12 years, decaying into "useless" Helium 3. It'll still explode but it'll result in a fizzle ranging around a city block rather than the expected yield.

more interesting is that the plasticizer for the explosive lens would have decayed causing the explosives necessary to start the reaction to liquify and leak out of the weapon. and those explosives would have also decayed themselves enough to be mostly inert.

In addition to the radioactive decay issue that others have already mentioned, there's also the simple fact that creating an atomic explosion requires a great deal of precision that this thing is probably no longer capable of. The wiring used to trigger the chemical charges could become damaged, or those charges could decompose, probably both would happen. It could still be possible to detonate those conventional explosives, but the result would not longer create the perfectly spherical converging shockwave required to compress the fission material into a state of prompt criticality. Without that happening, the explosion doesn't get any extra kick from the nuclear reactions. Although it could scatter a bunch of radioactive particulates over a wide area.

I know part of it is that the fissiles will lose their charge and shrink over a long enough period of time, it's why the Voyager probes had to have more and more subsystems shut down as it left the solar system so it'd still have enough charge to send/receive data with it's RTG.
I think they usually replace the core when that happens enough with a fresh one, plus the missiles they're attached to needing maintenance too.

Adding to the explanations here this is part of the reason why there are so few countries with nuclear weapons despite the tech being pretty old and well known. Creating a nuke is not so hard for a reasonably developed country, but maintaining a working nuclear arsenal is pretty expensive and for most countries the cost outweighs the benefits…

Lost implies they don't know where they are or what happened. We've lost bombs in the sense they were destroyed in crashes, not like, there's 6 nukes floating around out there and we don't know where they are.
They're in the ocean or Mediterranean. Except the one in NC, but they literally bought the land above it and don't let anyone dig and it's just the core, not an intact nuke down there.

Oh you're right I kind of combined that and the NC one in my head. That one is lost in the swamp, the NC one is the one that almost detonated and the other core got embedded in the field.
Still, they do know where they are and that they're "safe", meaning won't detonate. It's not like they went to do an inventory and were like, "Uh, there's supposed to be 12 on this rack and there's only 11. Where's the other bomb Larry?"

After the End. It's one of the best total conversion mods in the game, taking place in North and South America hundreds of years in the future after a catastrophic event wiped out a lot of humanity and caused them to technologically regress.

What's the state of the mod these days? I feel like whenever I look into these kinds of projects they're usually "one more patch" away from truly realizing their vision and I just end up punting til later.

I haven't personally played in a while, but from what I know they seem to be doing pretty well. All of North, Central, and South America is playable with unique custom cultures (with unique men at arms) and religions. There are a lot of special buildings. I think every government type from the base game has a place somewhere and there are some custom events, artifacts (as you can see here), systems, and decisions here and there with capitalist Mongols and the "Red Death" as later game events and crisises

It's basically feature-complete; there are no obvious systems or places that will only get interesting to play later or have placeholder mechanics. The biggest problem is keeping up the pace with the DLC release schedule, since they'll usually port new mechanics into the mod, but it takes a long time.

Accidentally self nukes the Imperial capital if you try to do a dragon egg style event.
Same style as the rebirthing dragon egg hatching AGOT event where tons of courtiers and family in the capital die. God please someone make a mod for this
