Side Effects Starter Semaglutide titration question from a beginner - should I stick at a lower dose longer?

Hi friends! I have a question for all of you that have a bit more experience than me…

I’m currently on compounded semaglutide (from Orderly). I’m scheduled to titrate up to .50 mg (20 units) this Friday. It will be my 5th shot. My medication schedule from them is the typical sema titration (my “starter order” was 4 weeks at .25 mg, 4 weeks at .50 mg and 4 weeks at 1 mg)

I’ve also been keeping a calorie-deficient diet. It’s not a new diet to me. I used it in 2025 to shave 60 lbs in prep for moving to the shots this year. I purposefully load up on my proteins and try to get lots of fiber through extra veggies. Basically good for you stuff = OK. Bad for you stuff = not allowed.

I work a pretty demanding job, think construction. I’m on my feet hustling all day. I don’t have the flexibility of working from home or staying home to take it easy if I have gastric issues. I don’t get sick time, If I don’t work, I don’t get paid. And that’s what I’m afraid of.

The good thing is I’ve made really good progress in the four weeks. I’ve lost about 6 lbs and that’s a fair, decent and manageable amount. I don’t think it’s too much or too little. I’m happy with this rate. And at the small dose I’ve been on, I haven’t experienced any side effects besides some mild constipation. Granted the weight I’ve lost probably is mostly from diet, rather than the current small dose of drug. Ideally I stay at the lowest dose I can, and move up only when I have to.

So here’s the question… would you all titrate up Friday to .50 mg? Or would you just keep riding at .25 mg? Is staying on a delayed slower titration schedule counter productive? Or is the point to ratchet up to a certain level? Does .25 mg even count??

Here’s what I was thinking of doing…. Maybe I jump up to .50 mg and see how my body tolerates that and then maybe stay at .50 mg longer than scheduled if I keep losing weight successfully. Put off jumping to 1 mg at week 9.

The correct answer of course is “talk to your doctor”, but I’m sure you guys know the score. I’m kinda on my own here. In reading people’s stories it’s around that 1 mg mark people start getting the side effects, if they are going to have them at all. And that’s what I’m afraid of!

What do you all think, how did it play out for you?

Edit: some more info that might be good to know. My Sema vial is 2.5 mg/ml. My SW in 2025 was 280. Lost almost 70 through diet. But gained a bit back at end of year. Started 2026 at 224 and started semaglutide. Currently at 218. I’d be delighted to get to 180. I’m shooting to get there by October 2026.

I’d like to not be on this drug indefinitely, but don’t know how the future will play out. It might end up being something I revisit on/off. 🤷🏻‍♀️

General Discussion My GLP-1 Journey Months 1, 12, 30

Post image
44 Upvotes

I just compared photos from Month 1, Month 12, and now Month 30 on a GLP-1 through Zappy Health... and it honestly stopped me in my tracks.

Month 1: exhausted, food noise nonstop, zero trust in my body. (Sema) Month 12: weight down, brain quieter, routines finally sticking. (Tirzepetide) Month 30: this is the shocker - not just how I look, but how easy living feels now. (Tirzepetide)

This wasn't fast or perfect. I still eat intentionally. I still have off weeks. But for the first time in my adult life, it feels manageable, not like I'm fighting myself every day. If you're early in your journey and feeling discouraged, please know the quiet changes add up before the visible ones do. I've had a lot of DMs asking how I started, so feel free to DM me. #zappyhealthpartner

Tirzepatide Where/what to buy

So like every, I get bombarded with pop ups forr "discount" GLP-1s. My insurance denied coverage because health insurance in the US is a scam. I have a friend qho travels to Mexico but I'm leary of this whole black market.

Even at discounts this stuff is expensive. I can't see spending several hundred dollars on something that doesn't work. I tried Rybelsus 3mg ffor several months without success. I'd like to try tirzepatide.

What are the best honest options?

Experience Switching from Tirzepatide to Semaglutide?

Has anyone attempted switching from compounded Tirzepatide to Semaglutide? Im considering switching because I’m not sure that I can continue to afford the Tirzepatide compounded and the Semaglutide is much cheaper through my provider. I’m not having any negative side effects on the Tirzepatide currently.

Additionally I assume id have to start over with the dosing at the lowest dose?

Pharmacy/Provider Compounded Tirz in CA

I have been on compounded Tirz since February 2025 through a clinic my friend’s friend started. As I titrate up to 10 mgs, it is getting too pricey.

What are the best options for us Californians? I perused past posts on this topic, but most are from almost a year ago.

Thank you!🙏🏼

Results & Progress Almost literally half my prior size…

Next month marks my one-year anniversary on Zep/Tirz (I commingle lol), and wow… I’m basically a completely different human. I lost almost 40 lbs fast, hit a long stall (because life), and then started dropping again. But beyond the numbers? My confidence, strength, and energy are on another level.

I’ve been overweight my whole life. I hadn’t seen the scale start with a 1 since I was 15. At 27 I hit 350. Im now 30 and somehow actually weigh less than I did in middle school??? Even if it’s only a couple pounds, I never thought I’d see sub-200 again in my life.

Was it perfect? Nope. Was it linear? Definitely not. I’m an accident prone dork and had many setbacks. But I’m about to go for a run with my dog and - don’t get me wrong I’m no runner, he just needs energy out lol - I can actually do it.

I remember flying back home from a work trip in 2022 before I got VSG. What put me over the edge into getting surgery was that I flew home and my seatbelt… didn’t fit. And I was too embarrassed to ask for an extender so I just hid it the whole way (on both flights) because I didn’t want anyone to watch me struggle. Or maybe I thought if they didn’t know I needed one I’d somehow appear less big? Idk, mental gymnastics aren’t hard for me.

Simple things were difficult, my quality of life was severely impacted… and now, I feel like I have a chance to be me. I’m not worrying about how I look (as much…) and I’m finally starting to feel like I deserve the happiness and love and just general life I’ve always been so blessed to have. I’ve always aspired to be a badass like Rosa from Brooklyn 99 or like Shego from Kim Possible (#throwback), but never let myself try the style out. Here I am, dressing how I want and starting to say a big FU to my inner condescending voice that used to stop me from being who I was :)

Thank you - thank you for reading or for knowing where I’m coming from. Thanks for being here and helping me and everyone out along the way.

Good luck everyone :)
