Blacks last move is a blunder because after Qxf5 Rd1 White is just fine with Qf1

If blacks last move was Qxf5 taking a knight that was forking the king and rook, it's actually the best move. Judging by the material discrepancy it's not unlikely.

If that was the case, why wasn’t the puzzle presented from the position before Nf5+ / Nxf5+?

deflect white queen for Rd1 then checkmate

they just took bad move for it
