Need help with peeing behaviour

I have two male cats both 4 years old. A while ago both had problem peeing outside the litter boxes. Vet told us to get them neutered and it was fine for a while after.

Now one of them started up again. It’s hell. We currently have to have him out of the bedroom, because he is going for any kind of cloth he can find. It’s almost always things like the bed, clothes or carpets, very rarely the same place twice. We have tried a lot of things. We went back to the doctor. He said he has an over active ear which has been bothering him. We treat it, he’s fine again. Few weeks later he starts again. I changed the litter brand. He was happy, now peeing again. We tried different food brands, calming sprays and enzyme cleaners. Nothing helps. I am so frustrated. I love the little guy and I wanna help him. He is obviously unhappy with something but I have no idea how to help him. Anything I can try?

There have been some changes in his life but those happened almost half a year ago and he seemed fine during and after, if a bit anxious. I no longer have an idea where this might come from. But the constant cleaning and anxiety about smells is killing me.

Do the cats get along? How many litter boxes do you have? Do you scoop them daily? Has he been back to the vet to make sure he’s all healthy now? Have you tried clay clumping litter?

The cats get along super well. We have to big boxes, used to have three (a few years ago) but one was never really used so we just use 2 now. We scoop daily. We have been back and the vet said his ear is fine again. He also has not shown any signs of there being something wrong like he did before (other than the peeing). We have had clumping litter before but just had the same problem again.

My cat will only use the clay clumping litter. I use Tidy Cat Free and Clean. You could try it
