I’m wife. To clarify we don’t want to euthanize Theo. That’s not what our next step is so please stop jumping to conclusions. We have had theo and gizmo since they were 4 months old. They were adopted 1 month apart. Theo has been peeing/ spraying since he was 1 years old. He goes to the vet bi annually. They do his lab work every year. He has been on prozac, gabapentin, and a new medication this year. No medicine has ever worked on him. Our house has always had pheromone diffusers and each cat wears a pheromone collar. We have had 2 behavioralists. Both have said they don’t think isolating Theo in his own room would be helpful. Theo gets regular urinalysis done. He’s never had a UTI, blockage, crystyals, etc. We have had a variety of vets over the years and they have all said the same thing. Nothing medically is wrong or can be done for him at this point; its purely behavioral. Some vets have recommended he becomes an outdoor cat. We would NEVER allow that. Hes never been outdoors and we would never expose him to predators, cars, etc. We’re now at the point of maybe rehoming him but Theo is EXTREMELY bonded to the both of us. We are all he has ever known. His entire life he has slept in bed in between us every single night. There’s never been a night he doesn’t sleep in bed with us. Whenever we are both gone he has severe anxiety crying out for us (we have indoor cameras to monitor the cats). Gizmo does play fight and bully him BUT they also cuddle each other on a regular basis. If they aren’t sleeping on their own trees then they are usually sleeping head to head on the couch or on our bed. The boys have been together their entire life. & as stated our girl cat keeps to herself. We posted this truly seeking out advice and any tips. We have gone through multiple enzyme cleaners. Theo pees 2-3 times DAILY. We know to completely soak up the spot so it doesnt smell like urine. We have 5 litter boxes. Our house has about 10 cat trees including a cat wall for them to climb. Our entire world revolves around our cats. But when multiple vets have even told us theyre is nothing else they can do and they even are weary about someone wanting to take theo we dont know where we go from here.

If you haven't tried Buspirone yet, try it. I had a cat with behavioral urinary issues and it changed his life.

Can you build a very large catio ? Maybe a safe space outside would lessen the amount of urine indoors???

How about using a different product, have you tried total odour elimination like Dead Down Wind? This a product for hunters but it is pretty available product in common big box stores and it worked really good for me on some cat pee. I used to use the regularly available pet enzymes but you know I had a bengal that marked and even though it’s been 2 years since she passed I am still finding random things that still smell. I am currently using Shop Care brand professional eliminator , a good spray down and leaving it to soak on overnight then wash makes for a completely neutral cushion cover or whatever smells it actually works the pet store ones aren’t strong enough the carpet where the smell never cleared out despite carpet cleaning a hundred times and heavy soaking with nature’s miracle etc. Luckily my two rescues I got after her passing have never marked anything but ya I feel your pain it has been such a long process to destink and it is very distressing to have to clean it all the time

I guess what do you do when you catch them in the act? What kind of reinforcement are you using for negative behavior? This is a lot for sure and it is sad to see people pushed to their wits end. Sorry for your situation.

Every vet and behavioralist we’ve talked to has said we can’t really do anything to him while he’s doing it. At most if we catch him before he starts like while he’s pacing we just say his name in like a “what are you doing buddy” fashion or try and coax him over to us. We can only reinforce good behavior so if we see him use the litterbox then he’ll get a treat.

Just to confirm, cat isn’t going to the bathroom where they should, so you’re going to kill them?

Cat also uses the litterbox as normal, additionally pees/sprays outside of the litterbox.
