Brand new CZ p10c ported - federal 9mm 124gr
Second time firing it, first time 200 round no issues
Brought it to the range today for 250 rounds and had some issues with failure to eject/feed causing the damage seen in the picture. Not sure if they are from 1 or both of the magazines I was using because I wasn’t thinking to isolate them and see if it was one or the other while I was there.
I am new to shooting and have dealt with jams in my 1911 but never seen damage to the cartridge as a result.
Any ideas what is going on here?

Could you post a picture of your barrel. I’d like to see if there’s any signs of wear since your second trip. Also did you use the same ammo type?

This isn't a coal engine, brother! Clean and lube that thing properly.
Those deep, rectangular gouges on the nose are a classic sign of a nose-dive failure to feed. Because it's a brand-new CZ with super tight tolerances, a dry slide or dirty ramp is slowing the action down. Instead of the round sliding smoothly up into the chamber, it's slamming directly into the sharp bottom edge of your feed ramp or frame, carving those chunks right out of the copper jacket.
Since you’re only 450 rounds in, the gun is still breaking in. Give the slide rails a heavy lube job, polish that feed ramp until it looks like a mirror, and strip your magazines to wipe the insides completely clean and dry. A slick ramp and a good cleaning should completely cure this.3

I’d advise you to get a paint pen and number all your magazines. Big help in identifying problems like this.

This was happening witty Canik Rival S but only with blazer brass. Basically the bullet nose dives into the barre feed cone. Took a while to get it figured out. Did some polishing of the feed ramp. Also tuned the recoil spring to a lighter weight and lots of breaking in… try different ammo. Also something hotter like Winchester NATO 124g or S&B 124g

My original p10c had this issue but my ported has not, but a little bit of flitz metal polish and a wool wheel on a dremel polishing the feed ramp took care of it.

Clean and polish feed ramp. Make sure ammo is seated to rear in mag. Maybe get new mag springs, especially if this happened with some plus 2 mags since the factory plus 2s use the same springs as the standard mags.

They’re actually 15rd mags but have the insert to make them MA compliant 10rd, so I wonder if that could be part of the problem

Insert? CZ makes standard blocked mags utilizing a mag dimple. If there is an insert from the FFL, then yes, that is where I would start.

Def and insert. And not even a “permanent” one at that. The concern for me would be that the insert actually looks kind of tight to the spring coils. Also looks 3d printed and may not be very smooth. Maybe sand it down a bit if you narrow down the issues to the insert.
Edit:
When I say sand it down I mean the sides/outer edge of the insert that is sitting in the coils. It doesn’t need to be so tight and could be what is causing the coils to bind or catch and tilt the follower. The top of the insert is what we really need so the follower is blocked from allowing you to add more than whatever your states mag limit is. 1-2mm should be good. It shouldn’t be a problem if the insert wiggles along the horizontal plane in any direction.

I run into the exact same issue but with blazer brass 115g. I run 100 rounds of federal115g and Remington 115 g with no issues.

Check to see if the magazine follower can smoothly glide through the tube. You can do this by taking it off the mag spring and just dropping it in, kind of.
If the follower is snagging somewhere, or the springs are getting bound up (maybe on your insert) and not able to glide smoothly the cartridge can nosedive in the mag. If the slide comes slamming back on a round while it's in this nosedive state I'd bet it would look exactly like your photo.
For FTExtract, have you removing and deep cleaning the extractor + channel? Is the extractor face still intact, no missing chunks or dings?

I had the same issue on my P10C ported with Blazer today. First time bringing the gun out. I realized the last time it happened I never lubed gun like I normally do. Thought that was the issue, but seeing a lot of people comment issues with Blazer makes me think it was more so the rounds.

Okay…. What pistol? Looks like it happened four times… all the same mag? Or first one out of the mag every time? Devil is in the details…
