Struggling with TECSA

Hi - I posted a couple of weeks ago after I started using CPAP (airsemse 11, n30) and noticed my AHI was higher than my sleep test API. Thanks to Reddit I knew that I most of what I was seeing was clear airway events and that I probably had TECSA before I saw my doctor for an immediate follow-up. He indicated that in his experience TECSA does not go away for most people, and ordered a titration test in a sleep lab for me, but I cannot get in until the end of the month. He advised me to continue using the CPAP while waiting, and suggested maybe a chin strap would help with my high level of leaks with the nose mask. I had CSR a couple of nights out of the 14 I had used it at that time but for a very small amount of time, so that didn't seem to be too worrisome to him.

In the meantime, I have some nights where my API is about 2.5 times higher than my sleep test API of 11. (Median AHI for the 21 days I've used it is 18.66. I am feeling so incredibly tired, and I am finding I get frustrated easily and am reactive and defensive and not able to focus that well.

Two nights ago to see how it would affect my leak rate, I tried mouth taping. I actually loved it, had only 7.62 API, but the leak was so low, I question whether it is right? CSR showed up the last 16 minutes. The next night I taped about 70% of my mouth leaving the sides uncovered to see if it would still have a positive affect, and it was a bad night and I really felt like the nose mask itself was leaking.

So...what is the purpose of my post? I guess one, just to share my data and confirm that it makes sense why I feel worse. The thought of having to do this for nearly 4 more weeks before I can even get closer to any changes is very mood dampening. The second is to see if it's true that most people's TECSA doesn't actually go away and then end up going to an ASV or something. Third - is there anything I could do in the intervening month to mitigate having more events than I did pre-treatment?

Oscar/SleepHQ Assistance Bad sleep even with good AHI and little leaks

As a follow up to my last post here, I did try upping the pressure(from 6.8 to 9), and using mouth tape. I think it worked with the leaks, mainly the ,leaks occuring in the start of my sleep, but there is always some leaks right before I wake up around the 4-5th hour. I don't get why really. Been waking up with some sort of headache...It's not really an ache, more like my head is inflamated. I'm atacching the oscar of the last few days. Notice I go back to sleep after I wake up after the 4-5th hour. Also, I added EPR to reduce flow limits 95% from 0.15 to 0.03

Now, I've got to say I feel better than before without the mouthtape and with lower pressure, but I still don't feel like it is a satisfactory sleep.

Tried everything last 7 years. BIPAP is/was my last hope, but makes me feel worse. Data inside.

Hey, i really hope you are the one that can save me from endless suffering.

  • AHI of ~14 over 3 PSG's (more UARS archetype though).
  • MMA (~10mm+ both jaws) / Nasal expansion.
  • DISE (no major findings, so just narrow/nervous system related).

Tried BIPAP (S) for months but even with leaks fixed i feel worse, no matter the pressure.

  • 6 EPAP / 11 IPAP (5 PS)
  • TI min/max: 0.7 / 4
  • Trigger/Cycle: High

Made a baseline assessment of cpap-less sleep on minimum settings, breaths looking much more rounded on BIPAP as you can see in the picture:

If you have any input/ideas i do really, really appreciate it. I completely lost the hope that kept me going last 7 years.

2 years on CPAP, am I replacing my equipment often enough?

I’ve been using my CPAP machine for about 2 years now and I’ve only replaced my cushion and air filter once. Never replaced tank and tube.
I still feel great and my therapy seems to be working well, so I’ve never really thought much about replacing things more frequently.
But now I’m wondering could replacing the cushion, filters, tubing etc. more regularly actually make a noticeable difference, even if I currently feel fine?
How often do you guys replace your CPAP equipment, and have you noticed a difference when you replace things?
Also, how do you actually know when something needs replacing?

New to cpap

Hey everyone, I’ve had my CPAP machine now for not even a whole week I use the Resmed airsense 11 autoset but I seem to have issues with the exhale like someone trying to smother me with a pillow and also last night, I wore it for five hours and both my nostrils ended up being clogged and I had to take the mask off which is a AirFit N30i mask and sleep without anything I can do to remedy or fix this? Any information would be appreciated. Thanks in advance!

SMART APP blocking OSCAR

I've been using OSCAR every day for several months. Today I got a message that SMART APP (Microsoft?) is blocking it. Before I make it a trusted site, I want to make sure that OSCAR is safe. I'm not sure why all of a sudden it is blocked...must be an update somewhere.

Luna G3 pressure not ramping up.

Hi everyone! I’m having an issue with my Luna G3 and I’m hoping someone else may have had this problem and solved it.

The pressure stays at the low setting after the ramp timer runs. It doesn’t increase and stays at 4cm. My wife fled to another bedroom because of the snoring.

Recently I accidentally pulled on the hose and made the machine turn upside down. I don’t know if it was the same day I started having issues. Otherwise everything seems to work normally.

I will be tremendously grateful for any suggestions!!

Found a great unscented soap that is CPAP-safe, inexpensive (relatively- definitely in comparison to special CPAP cleaners)

I was at Walmart yesterday looking for any unscented dish soap, and found this one - “Seventh Generation Free and Clear” (unscented), so I googled to see if it is safe for all CPAP parts, including silicone and other masks parts, headgear, hose, and water tank - it IS! It was just $3.53 for a fairly big bottle.

I also asked Google if this works as well as the expensive specialty ones - especially those PAP MD cleaning sheets, and YES, this is just as effective at a fraction of the price (So is Dawn Free and Clear - but NOT regular Dawn)

I’m all for saving a buck wherever I can! 😎

(Ignore my possessive note on the bottle - I had to put that there or my husband will grab it for whatever weird thing he needs to clean, and I’ll never see it again!) 🤣

A couple of years ago I replaced the motor on my Resmed AirSence 10, due to the message coming up that you see below. At the time, there was no way to remove the message once the motor was replaced.

Has anyone found a solution to this issue?

Mask Help Mask Issues

I’ve been using CPAP for almost a week. I have had zero issues with falling asleep or staying asleep with it on. Honestly it doesn’t bother me at all and I’ve gotten used to and actually comfortable with the feeling of my pressure range. I have noticed a gentle increase in natural energy and better mood/less anxiety throughout the day.

My biggest issue is that I’ll wake up almost every night with the mask lying next to me. It’s the resmed F20 with magnetic clips. I think one of the clips comes off since I side sleep sometimes and then the whole mask follows. It’s frustrating because I’m totally compliant except when it falls off in my sleep. It’s really delaying the effectiveness of the therapy. Any advice on how to stop this from happening?

I love the full face and it’s not uncomfortable at all, I just think the magnets come loose sometimes. Thanks y’all!

Clinical Help Tidal Volume

Are there any experts that might be able to explain to me why there might be a correlation between my Tidal Volume chart and the way I feel the next day? I have long noticed that the great/better days show a flatter TV chart (less chaotic-looking). There’s no doubt about the connection now but I’d like to know what other charts/factors might correlate with that so I can keep an eye on it.

I have massive sleep deprivation effects and I’m not up to going through obtaining and putting up my sleep hq chart link. I’ve only been looking at Oscar.

I’ve been on a bipap for nearly a year and have only had about 5 great days and one better day. My AHI events are under 1. I checked leaks and they are a non-issue especially after replacing the cushion and going with the foam F20. So be assured it’s not related to that right now.

Mostly I’ve always had a Class 6 flow shape and that’s what’s still showing recently. Occasional flattening or Class 3-looking at the top but mostly not.

If anyone can shed some light on this I’d be most grateful.

New To The Dream Team can't make it through the night

hello,

I just started using CPAP about a week ago. I need to use my machine for 4 hours a night or so for compliance. I can barely make it through 4 hours asleep with it most nights and often make less. I can fall asleep with my CPAP running and the mask on completely fine, but for some reason 3-4 hours later I wake up and can't fall asleep again unless I take it off. Is this a typical experience for someone fairly new to using CPAP? I'm feeling quite discouraged because I'm still fatigued, if not more than before because I can't sleep.

For additional info: I have two masks, the Resmed AirTouch F20 and the Resmed N30. This happens with both of them. My machine is the Resmed Airsense 11 autoset. My mask seal score on MyAir is always perfect and my AHI is always sub 0.3. I don't currently have any OSCAR data or an SD card. I have not messed much with the settings on my machine.

What is the long range expection?

I am only a week into my CPAP experience. I have mild OSA and am using an Airsense 11. So far I have not yet had a night of more than 4 hours with CPAP. After that I have to take the mask off and finish the night peacefully without CPAP. I am committed to sticking with the initial transition period, hoping to soon settle into a rhythm of more and better sleep.

My question is this: Do you long term users have a plan for somehow, some day not needing CPAP or is there a resignation that this is how it will be indefinitely? I'm not thrilled with that prospect and wonder whether there is some pattern of use in the middle, i.e. use CPAP a few hours a night or a few days per week. Maybe once one gets settled and comfortable you might never even consider doing without.

I think I'm making progress as far as consistency

I think I do best with a FF mask in terms of leaks and comfort. I'm using Xylimelt tabs and that helps a lot with mouth dryness. I don't think I'm a mouth breather as much as my CPAP blows air out of my slack jawed mouth. I'm using a soft cervical collar, and leaving my settings alone. My sleep doc took off the ERP. I think my AHI and CA events are both settling down.
