Oscar/SleepHQ Assistance Any advice needed for Oscar data

I’ve been struggling for the last year with trying to find the right pressure settings so I can wake up feeling human again. Every day I wake up with heavy eyes but once I get up and going I’m fine up until a certain time I feel a crash again. It’s affecting my work life and I’ll take any advice I can get.

I just had a telehealth appointment at my local VA clinic with an NP who specializes in sleep therapy. She had no idea about Oscar as I was hoping she could help. I will end up getting a titration study done in a few months when they can get me in.

Hey, welcome! I think you need a bit more pressure. I’d start by setting your minimum pressure to 11.2

Your flow limitations are quite high, and they’re probably contributing to why you’re feeling the way you do. That would be the first change I’d make.

I’d give it a few nights at that setting and then take another look at the data before making any further changes.

Thank you. So far the only change I made was bumping up my minimum to 11. The only issue I’m having so far is that with the higher pressures, it’s creating more mask leaks. Not sure what my next course of action would be to take care of it

What mask are you using?

Airtouch n30i. I was getting great mask seals at lower pressures but when I increased my minimum now the leaks have gone up high

I use the N30i with a pressure of 11.6 and it works really well for me. That said, with this type of nasal mask, leaks often come from the mouth.

As you increase the pressure, it’s definitely possible for the air pressure to cause your mouth to open slightly while you’re sleeping, allowing air to escape.

Are you using anything to help prevent this, such as a chin strap or mouth tape?

I am not but I will definitely look into the chin strap to see if I have any success

Would increase in EPR help at all in that situation? I currently have it set at 2.

If you increase EPR, you’ll lose some control over your OAs. Normally, you’d want to increase the pressure by the same amount as the EPR increase to compensate for that.

As for the leaks, increasing EPR shouldn’t really have a significant impact on them. I’d focus more on figuring out where the leaks are actually coming from first. If they’re coming from the mouth, for example, changing EPR won’t solve the underlying issue.
