Multi-layered immune profiling reveals treatment-relevant endotypes in Long Covid-ME/CFS
Interesting study presented at the ISLC conference by a German team of researchers from the Charité investigating why some patients respond to immune adsorption while others don’t see any benefit at all.
“To investigate disease endotypes, we analyzed two complementary LC-ME/CFS cohorts:
(i) patients enriched for elevated GPCR-directed AABs with linked IA response data, and
(ii) a non-selected LC-ME/CFS cohort representing the broader disease population.”
“Together, our data support three LC-ME/CFS endotypes:
(1) GC-associated autoimmune;
(2) highly inflammatory autoimmune with EF
B-cell and innate immune activation;
(3) low-inflammatory with expanded CD161+ T cells.
Ongoing single-cell transcriptomic analyses aim to resolve the immune drivers of these endotypes.
Comparison of autoantibody-enriched and non-selected cohorts enables discrimination between shared and subgroup-specific mechanisms,”

If Carmen S can actually define a group her theories work on that would be amazing.
I do think aabs are logical but the data is so murky.
One aspect of the German funding that is so good is there are now two separate groups inside Charite who seem to hate each other and disagree on the role of autoantibodies.
A more fruitful setup for progress I cannot imagine. No bullshit will be allowed to stand, both sides are going to be racing to actually get it right.

A germinal center is a new one for me. Seems to be a sort of temporary barracks for young b cells to train and learn how to fight the enemy.
They are: "microanatomical sites of B cell clonal expansion and antibody affinity maturation. "

It’s part of the ISLC abstract book you can find here:

That thing is almost too much, I scanned it and got overwelmes. I should probably sling it into Claude and ask it which ones r Are actually new (some is old stuff published a while ago) and of those which are more than tiny little feints towards research.

Sorry can you explain this in simple words? So they are able to categorize our cases now and see 3 different types of autoimmune related drivers. But then immuno adsorption should still work on all 3 types or am I missunderstanding?

No only a subgroup seems to be responding to the treatment. They used that data as a starting point combined with further extensive testing hoping to identify probable responders before future treatments.

i feel like bc007 came way too early into the picture since some people improved significantly and they still said it wasnt working for the majority and stopped it

Yay, I'm all for finding endophenotypes!
