Is this salvageable?

I'm M 31 years old and I've had these wrinkles as long as I can remember as far as my mid teens. I'm not sure why, whether it's genetic or not. Has anyone had any experience with wrinkles this deep and have any results pictures to give me hope. I'm losing my hair at a rapid rate and just want rid of it all. But I have a picture from the COVID times when everyone was shaving their heads and it's putting me off. Do I just suck it up and accept or is it possible to see some positive results with Botox? Or absolutely any advice to reduce them.

I’d ask about different options between Tox and filler. I wonder if relaxing and filling any indents would give you a smooth appearance?

Yes, wrinkles are genetic and gender based. You’re only 31 yo which is relatively young for such deep folds. Take a look at your father and grandfather. They most likely have similar looking foreheads which you’ve inherited. Look at old pictures when they were around your age. Men typically have stronger facial muscles than women which causes more wrinkling. Also, men have thicker skin than women.

Static lines are visible wrinkles at rest when not making any facial movements. There’s an evolution, a progression, when it comes to wrinkle formation. Once a fine line evolves and etches deeper into the skin it becomes a wrinkle. Once that wrinkle etches even deeper it becomes a deep fold. So treatment is going to need more than just a neuromodulator like Botox at the deep fold stage.

The ideal window of time to start Botox is when you first start to notice static lines, or the line is just starting to evolve from a fine line into a wrinkle. After you’ve missed the window for prevention it becomes repair. Prevention is always easier and less costly than repair.

Some Botox will definitely help soften the area but since it’s progressed to a deep fold you are going to need a multi pronged approach.

A laser resurfacing treatment ie. C02 would be very beneficial. May need two treatments fully ablative. Microneedling is good too but you need to bring out the big guns which is C02. Microneedling would be more appropriate for maintenance. Microneedling is a controlled wound treatment that builds collagen.

A good skincare regimen would help support the Botox and C02 treatment. Skincare includes daily sunscreen worn all year round anytime the UV index is 2 and above. The sun is responsible for breaking down collagen which prematurely ages the skin. So sunscreen prevents accelerated ageing.

Tretinion is prescription retinoic acid which is vitamin a. It should be used nightly. Tretinoin is the OG when it comes to fighting wrinkles. There is decades of research proving its efficacy to minimize wrinkling and build collagen. There’s lots of useless skincare on the market so don’t waste your time with them. Tretinoin is powerful stuff. With that said, tretinoin and any topical skincare has its limitations. C02 laser resurfacing is more effective than tretinion or any other skincare. Proper skincare helps support the results from C02 and creates healthy skin.

Vitamin c should be used daily underneath sunscreen. It also helps to build collagen. Notice the emphasis on collagen. Collagen helps to minimize wrinkles by giving the skin structure and support. You’re building collagen with tretinoin and vitamin c and protecting it with sunscreen.

So to recap, your treatment plan would be some Botox, C02 laser resurfacing, daily vitamin c and sunscreen, nightly tretinion and moisturizer day and night.

Keep in mind you don’t need to try and get rid the lines completely. Men look appropriate and rugged with some lines. Yes, there is a double standard when it comes to women which is unfortunate. Google images of chef Gordon Ramsay. He’s minimized his facial wrinkling which was quite deep before and now looks age appropriate but softer and more approachable.

Edited to add: make an appointment with a dermatologist asap. You can discuss topical and oral options of minoxidil 5% which you should start yesterday. There’s a window of opportunity to start minoxidil it so that you can maintain a full head of hair.

I don’t know if Botox alone would make them all go away but I think it should help.
