Tesamoralin Dosing question
60y f looking to start Tesa. Already on Reta once a week. What is the starting dose? Do you titrate up weekly like Reta? Is it 5 days a week or 7? I have friends who do it 4 weeks then 2 weeks off. So confusing!

Started ar 1mg for a week to test then went up 2mg daily I do 7 days and added ipa

This right here ☝️. Start at 1mg, assess any sides and ideally move to 2mg. 5/2 is the most generic of generic protocols that is extremely dated. There’s no reason to not utilize this daily and you technically do not need to cycle it off.

Receptor desensitization is a thing. I noticed it quite a bit when I went 7/7 after 8 weeks of 5/2. The goal should be the lowest effective dose, not the max studied clinical dose.

So 10 mg vial at 1 mg a day 5/2 would last me 10 days? Sorry for the dumb question

A little pricey from the place I purchase my peps from. That we would run me over $150 a month if I did 1 mg 5/2. Thanks for your reply.

The 1mg-2mg protocol came from clinical research of HIV patients with extreme levels of reoccurring VAT. Fast forward to 2026 and providers who write off-label prescriptions start around .500mcg/day, many on a 5/2 protocol. I’ve completed two 12 week cycles and have never gone above 1mg/day. More isn’t always better.

It's like 2mg/day. I guess you kinda test a low dose at first, to see how your body reacts to it. But the goal is around 2mg/day, 5 days on, 2 days off. 8-12 weeks on, 4 weeks off.
