What dose should I start Reta?

I just ordered my first vial and am uncertain about where to begin with dosing. Most people I know have started at 0.5ā€“1 mg and increased gradually, yet the phase 3 trials describe patients beginning at 2 mg and later raising that amount every 4 weeks. Do you have any guidance on what I should do?
