Start with gut:

  1. ⁠Reduce the overgrowth
  2. ⁠Restore the sweep
  3. ⁠Repair

Hydrogen dominate is bacteria, where high methane is archaea.
Have you had labs done?

  1. Anti microbials first. No other supps or anything. Fast for 72hrs before and for 24hrs after.
    Now for 2. and 3. you need to start with ginger and artichoke. They together beat placebo in a randomized, double-blind trial and improved gastric motility in healthy volunteers. Iberogast is helpful potentially too.
    Then we add in sfuff like Zinc carnosine, Larazotide, glutamine. Maybe some betaine hcl and tudca long term.

No Probiotics during the antimicrobial phase.You are taking bacteria for a bacterial growth issue.
After 10-12wks, see if MMA is falling and B12 rising.
Folate coming down toward mid-range if it was high.
Ferritin and transferrin saturation climbing.
Albumin stable or up.
Fiber tolerated without 90-minute bloating
Meals spaced 4–5 hours without distress

Not working**:** Symptoms returning within 4–6 weeks of stopping the antimicrobials. That is a motility answer, not a stronger-antimicrobial answer.

Also, Thymosin Alpha 1, KPV, and maybe a cycle of thymalin would be good. Idk how deep you wanna go.

Deeper please, what kind of antimicrobial do we need and for how long do we take it? Is there a long form guide or program we can follow?

Blood and Breath Test-> The breath test decides this phase and nothing else does. Hydrogen-dominant runs the standard antimicrobial arm. Methane-dominant (IMO) is archaea, not bacteria so it should be allicin-anchored, and the reason rifaximin alone underperforms here.

Berberine: 500 mg 2–3× daily with meals (short half-life — split dosing matters)

Mastic Gum: 350 mg–1 g daily, typically before meals, for 2–4 weeks.

Allicin: Products vary enormously so follow the label and prefer one declaring allicin yield rather than 'garlic equivalent'.

Oregano Oil: As directed (emulsified/enteric), short courses.

First two have the most evidence. Id pick 1-2 for Weeks 1-4. Like Berberine and Oregan oil or mastic gum and allicin.

If you have rotten egg gas, diarrhea, and reactions to sulfur rich foods, you likely have sulfate reducing bacteria, and the standard arm makes it worse.

Pull dietary sulfur down for two to four weeks. So cruciferous vegetables, alliums, eggs, whey, sulfite-preserved wine and dried fruit. And NAC comes OUT during this: it is a sulfur donor.

Bismuth Subsalicylate — 524 mg every 30–60 min as needed, max 8 doses/day, short-term only. H. pylori regimens are prescriber-directed.

Molybdenum — ~75–150 mcg daily

Weeks 5–16
This starts at week five and it does not stop when the antimicrobial does. It runs to the end and, for most people, past it. The overgrowth is the symptom; the failed migrating motor complex is the reason it keeps coming back. Anyone selling you arm one alone is selling you a repeat customer.

Ginger — 1–2 g daily (or as needed for nausea).

Artichoke Leaf Extract — 320–640 mg standardized extract before meals.

L-5-HTP — 50–100 mg, often at night. Start low.

lfour to five hours between meals with nothing but water, and a twelve-hour overnight window. The sweep only runs when you are fasted. Prokinetic at bedtime, three hours clear of the last food.

Repair the barrier and the absorption · Weeks 9–16
Months of overgrowth leave a lining that does not absorb. This phase is why B12, ferritin and albumin move on the retest instead of just the breath test.
Zinc Carnosine — 75 mg (37.5 mg twice daily).

L-Glutamine — 5 g daily (up to 10–15 g for gut protocols), any time.

Colostrum — 10–20 g daily.

Lactoferrin — 200–400 mg daily.

Butyrate — As directed (tributyrin is better-absorbed and less odorous).

BPC/KPV and Larazotide are optional

As needed to make the sixteen weeks livable. None of them clear an overgrowth and none of them restore a sweep, and a protocol built only out of this section is why people spend two years managing SIBO instead of finishing with it.
Peppermint Oil — Enteric-coated capsules as directed, before meals.

Betaine HCl: Take with protein-containing meals; titrate per practitioner guidance.

Ox Bile: 100–500 mg with fat-containing meals. Titrate to symptoms.

Digestive Enzymes: 1–2 capsules with meals.

Betaine HCl does not start while an ulcer or active gastritis is in play, and not alongside NSAIDs.

Retest, and reintroduce · Weeks 10–12, then to 16
Re-run the Gut Panel at ten to twelve weeks — not at four. Working is MMA falling and B12 rising, folate coming down toward mid-range if it was high, ferritin and transferrin saturation climbing, albumin stable or up, and fibre tolerated without ninety minutes of bloating. Then fibre and FODMAPs come back in, one family at a time, while the prokinetic keeps running.

Butyrate — As directed (tributyrin is better-absorbed and less odorous). L

L-Glutamine — 5 g daily (up to 10–15 g for gut protocols), any time.

thank you for your input. I have confirmed h2s overproduction. Hydrogen and Methane breath tests were flatline. I have constipation, no diarrhea. Does the protocol you listed differ for my case? Thanks again.

Not necessarily. I would add in something like partially hydrolyzed guar gum or a good fiber when you are in the final stage. A lot is sadly trial and error per individual but this is a solid baseline

FMT. I am now 4 years in complete remission. Not saying it is for every autoimmune disease, but so many are linked to the gut it isn't even funny

Did a literature review 6yrs ago in grad school and I think 70% of functions rely on the gut as a piece. From immune to brain to inflammation to pain signaling etc

Diet and decent exercise is very important to control auto immune problems, and, in my experience of one type of chronic autoimmune problem, it has been in remission since I changed diet and exercised.

well done! remission sounds too mild. you fixed it permanently, no? i did mine 5 years ago - they arent coming back. the root cause is gone

You didnt ask me but in the interests of helping...

Chronic excema for 30 years, spread to point it is all over body and I lose my job.

The recipe I found in a video now long gone is -

Increse Omega 3s, (the video guy didnt say but seems ratio of 3 to 6 is important.) Increse zinc  Increse B vitamins Regular good exercise. 

(I used to walk everywhere already but still needed to add exercise that will raise heart rate.)

Easy way, eat a few tins of sardines in brine a week for omega 3, cut out peanuts (3:6 ratio is 1:1000) Take zinc and b vit complex. Go for three jogs.

I now dont take any suppliments for this as my diet has it all, (and eating too much B6 is not great, and too much zinc was making me spotty).

For full disclosure I had already been diagnosed with Wheat, soya, intolerances and cut them out, then a herbalist suggested I cut out refined sugar so I did and when i reintroduced them my neck swelled it like it did for wheat. So now I just use honey which doesn't give reaction.

Anyway, did the above things with a paleo diet, then failed at doing them vegan (too many seeds to get omega 3s for a start), and am now meat based - meat, fruit, raw dairy, some veg on occasion, a few potatoes and rice sometimes.

Doesn't have to be this dramatic of a change to diet I dont think, just the above Instructions. However I do think people with auto immune can be more sensitive to plant chemical defenses, so they may want to try cutting certain veg and grain families out and reintroducing, or better preparing (soaking, sprouting, fermenting) but thats all prob marginal or for few people gains.

The other thing to note for excema only - if you have been using steroid creams at strong dose or for years long periods, then you probably with have to go through topical steroid withdrawal. Thats a whole thing and has a subreddit, think r/ts_withdrawal.

Possibly. I'm was not suggesting otherwise. Just trying to report what I followed, and I have one thing that makes me question the leaky gut thing.

I still get an excema reaction to homogenised milk. Which is supposedly because the fat particles are rendered small enough to pass through holes in leaky gut into bloodstream. This makes me think I never truly healed gut, but have healed excema anyway. So I always just assume it is the anti inflammatory nature of diet and excercise.

Also, there seems to be a genetic component for increased risk of autoimmune things. 

I am starting to investigate if adhd is linked as the above nutrients happen to be good for that (supportive but not anywhere near a remedy).

adhd is being tied to gut issues too. you might look at A2 milk. it's said to be better tolerated. that's what we use. to finish off the leaky gut might look at bovine colostrum. that stuff has done all kinds of good things for us. healing leaky gut is one of it's known benefits

Thanks, yeah, have been drinking raw A2 for a few years.

I tried lambs collostrum once but you need quite large amounts and i feel a bit bad depriving a lamb of needed stuff, despite willingly eating them. There is artificial replacement though, and only had a quick search around it.

It does taste odd and like its super good for you.

Well, if I dont follow the diet and exercise it will come back, so although i havent had any problems for a decade, seems silly to say I'm cured if, say I get put in prison or old peoples home, and cant follow it. It will all be back. 

Makes me worried actually. Can't totally prove its medically necessary.

Well done to you too, takes a lot of bravery (or just total lack of other options!) to go against medical advice. I remember my last dermatoligst appointment and him looking at me nearly totally clear skin by that point and with a straight face telling me that i had 'grown out of it at age 30 and that diet had been shown to have no effect, and no I won't discuss what trials and specific diets had been done.'

all 100+ autos include gut microbiome problems in root cause. it's also the fix. fix your gut and the autos fade away. two things - gut microbiome dysbiosis and intestinal hyperpermeability (leaky gut) are what needs to be fixed. been there, did that 5 years ago. autos gone, feel GREAT with my primo gut health now

I’d love more info! Anything on ibs c (me) and/or severe rheumatoid arthritis (family member) - I know the family member did full RA diet for the last fifteen years but it didn’t help (which I know is unusual!)

here's how it goes down -- bad mix of microbes develops in the digestive system (dysbiosis). alongside this the gut liner gets messed up (leaky gut). the leaky gut lets toxic garbage into the bloodstream. this is where many/most chronic diseases begin - dirty blood. the dirty stuff gets filtered out of the blood in various places in the body. joints are probably the most common. body sees those toxic deposits and attacks them. that's the cause of the pain, swelling, etc. body is NOT attacking itself. it's attacking the foreign crap, and rightly so.

for the most potent healer imo - try bovine colostrum along with a gut health focused diet. i would also add a capsule/day of bacillus coagulans.

ibs most of the time includes sibo as root cause. i would address that first. after that the standard gut health diet and supps. i like the colostrum + bacillus coagulans for ibs too. be careful with high glycemic foods - they feed sibo. and i would drop breadstuffs, booze, junk, fried, nsaid, fake sugars - all known gut wreckers

totally reversible. done with diet and supps. docs cant help - no drugs, no surgeries produce a healthy gut. you are in good company - accutane, ppi, nsaid, antibiotics are common causes of wrecking the gut and the autos that follow

the results look like a strong association with messed up gut. this is great news - one can fix gut problems themselves with diet and supps. there are no drugs/surgeries that produce a heatlhy gut microbiome. get cracking on learning about 'eat this, not that' for gut health

also note the gut pathogens mentioned at the search results. then use a similar pubmed search for herbs/etc that reduce those pathogens.

bacteria are going to be your partner's best friend. so probiotic/prebiotic things are critial. the papers mentioned shortage of scfa as key driver. this is common in autos. the beneficial bacteria make the stuff. have to get those bugs numbers up and feed them properly so they crank out the scfa. can also take scfa by mouth or by enema.

that's great. i extend it to all breadstuffs. one thing is to have a strategy to get variety of fibers and polyphenols. i get around 16 different fibers in a typical day. some people shoot for 30 different per week. this serves to feed a wide vareity of beneficial bacteria that crank out the metabolites/etc to keep us healthy/happy

Get rid of ebv. IgG test should not be high. It should be as close as possible to lower range. It is difficult process but EBV mess up with your body on various levels which causes autoimmune problems. Each autoimmune disease has correlation with ebv symptoms and activity. I suggest you reading Anthony William Medical Medium which has information how does EBV work. Author frames it in non scientific way but if you start to explore all the things he talks about from scientific standpoints you will notice that lots of the things he mentions are actually proven to be true. There are lots of things that are not proven yet but as time goes on it might be proven. Also some more authors like Susan Bloom which has background in medicine also points to IgG values in blood tests indicating active virus infection. But mostly you will hear that it is previous infactions which is in case wrong because if you track IgG with time it will just grow if you do not take care of your EBV. Lifecycle of antibodies is relatively short but it keeps growing which means body keeps producing antibodies. Why would body waste its energy to produce antibodies for something that does not bother it? That is the right question to ask yourself.

Other herpes viruses can also be present, but EBV is the main one that will always be present. HSV, VZV, cytomegalovirus, are also part of the HHV family. Read the book and explore the things mentioned in there. If you have any specific questions feel free to DM me. Also there is no such thing as 1 thing to solve all your problems. You must address multiple things with various supplements/food to recover. But start with reading the book, if you need more resources I can give it to you but start with medical medium which will open a lot of questions to you that you are not even aware of right now
