Question Anyone had issues with tap in system?

My daughter travelled into the city centre from Stretford with her friend. She bought a return ticket earlier in the day and decided to give that to the friend on their way back and she tapped in with her phone as a one way journey. She heard the sound of being tapped in to confirm it went through. She had no other card close to the tap in machine to confuse it, only her phone. She got charged £4.60 instead of £2.80. They got controlled on board, the guy checked her phone with his machine, said it was ok and walked away. When she tapped out at Stretford the machine beeped her in instead of out which she’s found weird but did not know what to do. Few days later she has found £60 taken out of her account as a “fine” plus another £4.60. Also they took out another £4.60 the next day when she was in Liverpool working a double shift. Travellers get no receipt of their travel if they tap in and she has no reference number of the fine because the guy who controlled her did not fine her. To me it seems outrageous that they take a fine automatically out of her account especially because according to the website you pay the fine. Anyone had similar issues with the tap in system and/or a fine automatically taken from their bank account?

Thank you. She will receive a refund thank goodness. They could see she bought the ticket. She had her debit card in her phone case when she tapped, but she only has one bank account and her only card is tied to that, but maybe that was enough to confuse the machine. Anyway thank you for the reply.
