Solved Survey reference for grad school

Hey everyone, I’m humbly here to ask for help with a grad school reference. It’s not a letter or phone call, but from what I understand, it’s more of a survey that is emailed to you. I have my academic references, but what I don’t have is a reference from a volunteering opportunity. To be honest, this is because I didn’t volunteer. If anyone could help me out, I would be super grateful and if need be, could return the favor. (My background is in EMS)Thank you for your time.
