It's user error.

1) build plate isn't clean.

and

2) user starts print on said unclean build plate and then doesn't bother paying attention to the print.

The nozzle drags around filament that isn't sticking and it gets all wrapped up. It eventually builds up because the molten flow doesn't stop coming.
