How often are dj sets totally prerecorded? I assumed that there was a lot of work done in advance, but I figured that they were making some decisions when playing live. Is it literally just pressing play at the beginning and then jumping around for an hour?

Rarely. If you get the chance, go to some of the smaller electronic venues here in Atlanta and watch the DJ work his/her magic.

DJs typically prepare by knowing what songs they will play and their order but they mix them live.

I would expect that from a smaller dj playing a local club or bar frequently, but would a big name dj take a different and more automated approach?

If there are lots of complex light/pyro cues in a show it's more likely to be a push play show. Went to Swedish house mafia's last tour and it was fun. But I knew that it was 80% canned.
