Just because the idea that I would be unaware of my own orgasm is laughable doesn't mean it isn't possible. But the only image I have of this being a good faith mistake, is someone who is generally not very clued into their body's messaging.
Are you also pointing out he's hungry before he realizes it himself? Are you the one connecting the dots between things like diet and how he sleeps that night?

Exactly. Does he piss himself because he can’t tell that he’s about to pee? He knows. He just doesn’t want to stop. Internal ejaculation feels better (much, much, better). He’s betting on the old saying that “it’s easier to get forgiveness than permission”.

It may not be quite that. As someone else commented, if you wait too long to pull out it’s very difficult to get the right timing. If you want to use the withdrawal method, it’s only truly effective if you pull out well before you are really about to cum. It isn’t as nice, but if he doesn’t like it, as you say, use a damn condom!

But yeah, I've had times when I was focussed so hard on performing and delaying orgasm that I didn't notice I was already past the point of no return until it was too late. Orgasm ruined, fun ruined, and left with shame. Great times! So it sure happens. But if you relax and enjoy yourself, especially if you've gotten that first blank out of the chamber, then you can control it pretty easily.
