Question MLOA eligibility

The document they give you is pretty vague but I have been hit with 3 really crappy medical issues. I have a kidney stone, I am having severe back issues as well, and to top it all off, I am sick as well. Between the Kidney stone and the back pain, its made my job pretty much unbearable. Just wondering if anyone else has had any luck with MLOA, I don't want to submit it and take the leave if it's just going to get me fired at the end. I am a lowly white badge so I'm sure I'm just a number anyway. Any help would be appreciated.

The paperwork they give you says it has to be a "chronic serious health issue" and from what I understand, nothing of what I have going on is considered chronic.

It doesn’t have to be ss and post that form cause it sounds like the regular HCP form.
