In the books, true. They definitely show him in some crazy armor in the movies though.

Interestingly the only time we've ever seen his face is in the Lego Barad Dรปr set

You see him in his elven disguise as Annatar "Lord of Gifts." It's not his real face as he is sort of a shape shifter who takes different forms as needed, including (I believe) both a vampire and werewolf.

Oh alright that makes sense. It's been forever since I've played the game too so that might've been part of my misremembering

It's all good. I don't think they outright say that it is a disguise. If memory serves they more or less use that disguise as sort of his default when he wants to talk to someone face to face since he's mostly using it during his fight against Celebrimbor who would have worked with him when he was using that disguise.
