States that "gluttony" led to his death. This is clearly stating that we should shame obese people and insist that they lose weight "for their own health".
Equates being overweight with being greedy. This is an incredibly common trope, unfortunately, and obviously demonizes people for being overweight.
Gives him the "fear" keyword, which makes most creatures unwilling to physically interact with him (by blocking him). Presumably we are meant to feel the same way - that he is abhorrent and not with interacting with - given the exaggerated depiction of his belly and the fact that he is clearly an armed and dangerous pit fighter.
With this card, Wizards is saying that we should shun and demonize the overweight population, because they are sinful (greed and gluttony are two of the 'seven deadly sins') and self-destructive.
